摘要
目的:探讨治疗下肢深静脉血栓进行溶栓时尿激酶在不同溶媒中的疗效。方法随机选取2012年1月至2014年10月下肢深静脉血栓患者200例,随机分为 A、B 两组,在滤器保护下用小剂量尿激酶在足背静脉滴注(A 组尿激酶溶于低分子右旋糖酐中,B 组尿激酶溶于氯化钠注射液中),同时联合低分子肝素和华法林治疗,疗程结束后必要时取出滤器。观察患肢周径、多普勒彩超推测治愈率和有效率。结果 A、B 两组治愈率和有效率比较差异未见统计学意义(P 〉0.05)。A 组6例发生了速发型变态反应。A、B 两组均无一例患者发生肺栓塞、大出血或重要脏器损害。结论为避免使用低分子右旋糖酐引起的速发型变态反应,临床上可使用氯化钠注射液作为尿激酶溶栓治疗的溶媒,简单、安全、有效。
Objective To investigate efficacy of urokinase in different solvents on deep venous thrombosis of lower extremity thrombolysis. Methods Two hundred cases with deep venous thrombosis were randomly selected from January 2012 to October 2014,and were divided into group A and group B, the filter protection with lower limb dose of urokinase intravenous drip in the back foot,also combined with low molecular weight heparin and Hua Falin treatment,after the end of treatment when necessary, remove the filter. Observation of limb circumference,Doppler speculated that the cure rate and effective rate of ultrasound. Results The cure rate and total effective rate of group A and group B had no significant difference(P 〉 0. 05). Six patients in the group A had anaphylaxis. There was no patient de-veloped pulmonary embolism,bleeding or organ damage in the two groups. Conclusions In order to a-void the anaphylaxis induced by low molecular dextran,sodium chloride injection can be used as solvent for thrombolytic therapy with urokinase,and is simple,safe and effective.
