老年原发性高血压合并糖尿病患者降压药物治疗分析

Treatment analysis of antihypertensive drugs in treatment of essential hypertension with diabetes mellitus in elderly patients

目的：观察老年原发性高血压合并糖尿病患者降压药物治疗情况及效果，为临床合理治疗高血压合并糖尿病提供依据。方法选择2012年1月至2014年6月住院患者中≥60岁原发性高血压合并糖尿病患者80例，统计分析降压药使用及治疗情况。结果80例患者药物使用率：单一用药（23.75%）、二联用药（31.25%）、三联用药（33.75%）、四联用药（11.25%），单一用药中 ARB、CCB 用药频次较高；52.5%患者收缩压得到有效控制，舒张压控制率显著高于收缩压控制率；不良反应发生率为48.75%，病程〈10年的患者服药依从率为14.71%，病程≥10年的患者服药依从率为34.78%。结论老年原发性高血压合并糖尿病患者降压治疗以 ARB、CCB 药物为主，多需要联合降压治疗，临床需结合患者实际情况，监测不良反应，提高降压方案临床效果。

Objective To investigate the effect of antihypertensive drug on elderly hypertensive patients with diabetes,in order to provide basis in clinical treatment of hypertension and diabetes. Methods Eighty patients over 60 years of age with essential hypertension and diabetes were selected from January 2012 to June 2014,statistical analysis of the use of antihypertensive drugs in treatment. Results Medication usage： monotherapy （ 23. 75% ）, two drugs （ 31. 25% ）, triple therapy （33. 75% ）,quadruple therapy（11. 25% ）,ARB,CCB medication had higher frequency in single med-ication;52. 5% of patients with systolic blood pressure has been effectively controlled,diastolic blood pressure control rates were significantly higher than systolic blood pressure control rates;adverse reaction rate was 48. 75% ,duration more than 10 years of patient medication compliance rate of 14. 71% ,dura-tion less than 10 years of patient medication adherence rates to 34. 78% . Conclusions Elderly patients with essential hypertension and diabetes in antihypertensive treatment with ARB,CCB drugs,needs multi-joint antihypertensive treatment,to be combined with patient clinical situation,monitoring of ad-verse reactions,clinical program to improve antihypertensive effect.
