文拉法辛联合生物反馈治疗躯体形式障碍的对照研究

A comparative study of venlafaxine combined with biofeedback therapy in the treatment of somatoform disorders

目的观察文拉法辛联合生物反馈治疗躯体形式障碍的疗效和安全性。方法将56例躯体形式障碍患者随机分为研究组和对照组,研究组给予文拉法辛合并生物反馈治疗,对照组单用文拉法辛治疗,疗程6周。于治疗前及治疗后第2、4、6周末应用症状自评量表(SCL-90)躯体化因子分、汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)评定临床疗效,治疗中需处理的不良反应症状量表(TESS)评定不良反应。结果治疗6周末研究组总有效率高于对照组(P<0.05)。治疗后第4、6周末两组SCL-90躯体化因子分、HAMD、HAMA评分均较各自治疗前降低(P<0.05)。治疗后第4、6周末研究组SCL-90躯体化因子分、HAMD、HAMA评分均低于对照组(P<0.05)。两组患者不良反应发生率比较差异无统计学意义(P>0.05)。结论文拉法辛联合生物反馈可安全、有效地治疗躯体形式障碍。

Objective To explore the clinical efficacy and safety of venlafaxine combined with biofeedback therapy in the treatment of somatoform disorders. Methods A total of 56 patients with somatoform disorders were randomly divided into study group treated with venlafaxine combined with biofeedback therapy and control group treated with venlafaxine for 6 weeks. They were assessed with somatoform factor of Symptom Checklist-90（ SCL-90）,Hamilton Depression Scale（ HAMD）,Hamilton Anxiety Scale（ HAMA） and Treatment Emergent Symptoms Scale（ TESS） before treatment and at the ends of the 2nd,4thand 6^（th） week of treatment. Results At the 6^（th） weekend of treatment,the total effective rate in study group was significantly higher than that in control group（ P〈0. 05）. At the 4thand 6^（th） weekends of treatment,the factor score of somatoform of SCL-90 and total scores of HAMA and HAMD in both groups all decreased significantly compared with those before treatment（ P〈0. 05）. At the4 thand 6^（th） weekends of treatment,the factor score of somatoform of SCL-90 and total scores of HAMA and HAMD in study group were all significantly lower than those in control group（ P〈0. 05）. There was no difference for the incidence rates of side effects between both groups（ P〈0. 05）. Conclusion Venlafaxine combined with biofeedback therapy can treat patients with somatoform disorders effectively and safely.
