体重指数与女性乳腺癌的相关性研究

摘要 目的探讨体重指数(body mass index,BMI)与女性乳腺癌之间的关系。方法回顾性收集2013年6月至2014年12月期间经手术治疗后组织病理学确诊为乳腺癌的女性患者276例,收集患者术前的年龄、身高、体重数据,采集术后组织病理分级,临床分期及免疫组化等信息,临床分期采用乳腺癌TNM国际分期法;同时统计患者的既往史、月经及生育史。根据亚太地区肥胖的诊治标准,按照BMI的高低分为低体质量组,正常体质量组,超重体质量组,肥胖体质量组,严重肥胖体质量组5组。结果 276例乳腺癌均为女性患者,其中不同组别间乳腺癌患者临床分期上的差异存在统计学意义(P<0.05),超重和肥胖及严重肥胖组腋窝淋巴结阳性率高、肿块直径较大,且存在显著性统计学差异(均P<0.01)。同时超重和肥胖组的分子分型中三阴乳腺癌比例较高,差别有统计学意义(P<0.05),同时正常体重组较超重组luminal B型乳腺癌比例高,差别存在统计学差异(P<0.05)。结论超重和肥胖女性乳腺癌患者具有更差的肿瘤特征表现,超重和肥胖是女性乳腺癌的危险因素。 Objective To explore the correlation between body mass index(BMI) and breast cancer in women.Methods Retrospectively collected 276 female patients with breast cancer from June, 2012 to December, 2013 after postoperative histopathologic diagnosis, the data of age, height and weight of the patients were collected before operation. The data of age, height and weight of the patients were collected before operation, collected the information of postoperative tissues, the pathological grade, clinical stage and immunohistochemistry. Clinical staging of breast cancer were used by TNM international staging; the previous history, menstruation and reproductive history of the patients were also collected. According to the standard of diagnosis and treatment of obesity in the Asia Pacific region, and the level of BMI, the patients were divided into low body mass group, normal body mass group, overweight body mass group, obesity body mass group, severe obesity body mass group. Results 276 cases of breast cancer were female patients, there was significant difference in the clinical stage of breast cancer between different groups(P〈0.05). Axillary lymph node positive rate, tumor diameter were larger, and there was significant difference (P〈0. 01 ) in overweight body mass group, obesity body mass group, and severe obesity body mass group, the proportion of three female breast cancer was higher in molecular classification, and the difference was statistically significant (P〈0.05) in overweight body mass group and obesity body mass group. The proportion of normal body weight group was higher than that of B luminal type breast cancer, and the difference was statistically significant (P〈 0. 05 ). Conclusions Overweight body mass and obesity body mass patients with breast cancer have worse tumor characteristics ; overweight and obesity are the risk factors of breast cancer.
