双源CT对颅外脑动脉支架再狭窄的诊断价值

摘要 目的探讨双源CT血管成像(DSCTA)对颅外动脉支架置入后支架内再狭窄的诊断价值。方法选择56例实施颅外动脉支架置入手术患者进行DSCTA检查。两名医师独立对56例72枚支架的支架内再狭窄进行评估,并分析斑块性质。结果 DSCTA对颅外动脉支架内再狭窄诊断的敏感性、特异性、阳性预测值和阴性预测值分别为94.0﹪、95.7﹪、92.2﹪和96.8﹪。在25枚发生斑块的支架中,软斑块17枚(68.0﹪),混合斑块5枚(20.0﹪),钙化斑块2枚(8.0﹪),1枚同时出现软斑块和钙化斑块(4.0﹪)。结论 DSCTA可完整显示颅外动脉支架置入的整体情况,对支架内再狭窄的评估具有很大的优势,对明确支架内斑块性质有一定的诊断价值。 Objective To investigate the value of dual source CT angiography (DSCTA) in the diagnosis of re-stenosis for the patients with extracranial arteries stenting. Methods 56 patients with stent implantation were recruited and underwent DSCTA examination. Two physicians independently assessed the in-stent re-stenosis in 56 patients with 72 stents and analyzed the quality of plaques. Results The sensitivity, specificity, positive predictive value and negative predictive value of DSCTA in the diagnosis of in-stent re-stenosis were 94 %, 95.7 %, 92.2 % and 96.8 %, respectively. Of the 25 plaques in stents, 17 were soft plaques (68 % ), 5 were mixed plaques (20%), 2 calcified plaques (8%), 1 soft and calcified plaques (4%). Conclusion DSCTA can fully display the overall situation of the extracranial arteries stent, which can be beneficial in the evaluation of in-stent re-stenosis and identifying the quality of plaques in stents.
