摘要
目的 探讨超声在外周血管假性动脉瘤中的诊治价值。方法 选取2009年1月~2015年6月我院住收治的外周血管假性动脉瘤患者40例。采用常规二维超声及多普勒彩超检查,观察其超声表现及在多普勒彩超下进行压迫治疗的疗效。结果 二维声像图可见动脉旁搏动性囊性肿块,瘤壁无动脉壁结构。手指压迫供血动脉近端瘤体后可见瘤体缩小,搏动减弱或消失。多普勒超声图像可见瘤灶内有红、蓝各半血流信号,血栓附着处无血流信号;可见双期双向动脉血流频谱。40例患者均在多普勒超声引导下进行压迫治疗,治愈率为92.5%(37/40)。平均压迫治疗的时间为(36.2±3.1)min。治疗后复查多普勒超声,显示破裂口闭合,瘤灶变小,灶内被血栓填充,无血流信号。结论 多普勒彩超检查具有无创性,动态性特点,可清晰观察假性动脉瘤体、破裂口、瘤腔内血栓形成情况,现已成为临床诊断假性动脉瘤主要方法。超声监视下压迫疗效确切,安全可靠,且方便快捷。
Objective To investigate the value of ultrasonography in the diagnosis and treatment of peripheral vascular pseudoaneurysm.Methods From January 2009 to June 2015, 40 cases of peripheral vascular pseudo aneurysm in our hospital were selected. Using conventional 2 dimensional ultrasound and Doppler ultrasound examination to observe the ultrasound performance and the effect of compression therapy under Doppler ultrasound.Results 2 dimensional ultrasound images showed that there were no arterial wall structures in the aneurysm wall. After the finger pressing the blood supply artery of the proximal end of the pseudoaneurysm, the pseudoaneurysm can be reduced, band the pulse is weakened or disappeared. Red, blue half blood flow signal Doppler ultrasound image can be seen within the pseudoaneurysm and thrombus attached at no blood flow signal; Visible double bidirectional artery blood flow spectrum.40 patients were treated by compression therapy under the guidance of doppler ultrasound, and the cured rate was 92.5% (37/40). The average treatment time was (36.2±3.1) min. After treatment, Doppler ultrasound showed that the pseudoaneurysm was closed, and the pseudoaneurysm size was small, and there was no blood flow signal in the lesion.Conclusion Doppler ultrasound examination with non invasive, dynamic characteristics, can be clearly observed in the pseudoaneurysm, rupture, aneurysm cavity thrombus formation, has become the main method of clinical diagnosis of pseudoaneurysm. Ultrasonic monitoring of the treatment effect is exact, safe and reliable, and convenient and fast, it is worth to recommend the use of a clinical method.
