临床药师对糖尿病患者提供临床药学监护的效果

The Effect of Clinical Pharmacists on Providing Clinical Pharmacy Care for Patients with Diabetes Mellitus

目的探讨临床药师对糖尿病患者提供临床药学监护的效果。方法对临床药师按照时辰药理学、药物与药物间相互作用原理及药物与食物间原理进行介绍,为1例老年糖尿病患者提供用药指导,制定合理用药方案,并全程跟踪,统计治疗效果。结果根据临床药师制定的"服药时间指示图"调整用药时间,治疗效果明显提高,且患者用药依从性也提高;经调理后,患者血糖水平、尿酸水平和血肌酐水平明显改善,病情稳定后出院。结论根据患者的病情临床药师与患者沟通,为患者提供用药指导,可提高患者临床用药的依从性,对最大限度上发挥药物的临床治疗作用具有重要的意义。

Objective To investigate the clinical pharmacists to provide patients with the clinical effects of pharmaceutical care. Methods The clinical pharmacist in accordance hour pharmacology, drug-drug interactions between drugs and food between the principle and the principle introduced to provide guidance for one case of medication in elderly patients, to develop a reasonable regimen, and tracking, statistical treatment effects. Reults According to the clinical pharmacist to develop a ＂medication time indication map＂ time to adjust medication, the treatment effect is significantly improved, and also improve patient medication compliance; after conditioning, blood glucose levels, serum creatinine and uric acid levels significantly improved, in stable condition after discharged. Conclusions According to pharmacist clinical condition of the patient to communicate with patients, medication guide for patients, improve patient compliance in clinicai medicine, and has im- portant implications for clinical treatment effects of drugs exert the greatest extent.
