当归六黄汤联合西药治疗阴虚火旺型肺结核60例临床疗效观察

Curative Observation of Combining Danggui Liuhuang Decoction and Western Medicine in the Treatment of 60 Cases of Yin Deficiency and Fire Hyperactivity Type Pulmonary Tuberculosis

目的:观察当归六黄汤联合西药治疗阴虚火旺型肺结核的临床疗效。方法:将60例阴虚火旺型肺结核患者随机分为对照组(30例)和治疗组(30例)两组,两组均接受2HRZE/4HR化疗方案治疗,治疗组加用自拟加味当归六黄汤,观察两组治疗前后痰菌(痰涂片)、肺部结核灶(X线胸片)、肝功能损害和临床证候的变化。结果:经治疗后,治疗组总有效率为89.3%(治愈9例,显效8例,有效8例,无效3例),对照组总有效率为64.3%(治愈5例,显效7例,有效6例,无效10例),治疗组疗效显著优于对照组(P<0.05);治疗组治疗后痰菌转阴率,肺部结核灶吸收明显提高,咳嗽或咳痰、咯血或痰中带血丝、五心烦热或潮热盗汗等临床症候显著改善,肝损伤发生率降低,且明显优于对照组(P<0.05)。结论:当归六黄汤联合西药可显著改善阴虚火旺型肺结核临床症状,明显提高疗效。

Objectives： This study aimed to explore the clinical effectiveness of combination of Danggui Liuhuang Decoction and western medicine in the treatment of hyperactivity of Fire due to Yin-deficiency pattern of pulmonary tuberculosis. Methods：Total 60 patients with hyperactivity of Fire due to Yin-deficiency pattern of pulmonary tuberculosis were randomly assigned to controlled group（ 30patients） and treated group（ 30patients）,and the sputum smear,focus absorbing,liver function impairment and TCM symptoms for these two group were also observed. Results： After treatment,the total effective rate of the treated groupwas 89. 3%（ 9cured patients,8excellent patients,8valid patients,3invalid patients）; the total effective rate of the controlled group was 64. 3%（ 5cured patients,7excellent patients,6valid patients,10 invalid patients）. The clinical effectiveness of the treated group was higher than the controlled group（ P〈0. 05）. In addition,the positive rate of the sputum smear,focus absorptivity were notable enhanced,the clinical signs were significantly improved and the liver function impairment were notably decreased in the treated group,and the effectiveness of the treated group was higher than the controlled group（ P〈0. 05）. Conclusions： Treatment with combination of Danggui Liuhuang Decoction and western medicine can significantly improve the clinical signs of hyperactivity of Fire due to Yin-deficiency pattern of pulmonary tuberculosis and notably enhance the clinical effectiveness.
