通窍活血汤对短暂性脑缺血Lp-PLA2水平及糖脂代谢的影响

Effect of Tongqiao Huoxue Decoction on Lp-PLA2 Level and Glucolipid Metabolism in Patients with Transient Ischemic Attack

目的:观察通窍活血汤对短暂性脑缺血(TIA)发作患者Lp-PLA2水平及糖脂代谢的影响。方法:选取2012年12月至2015年12月期间我院神经内科收治的TIA患者79例,以随机数表法分为观察组45例,对照组34例,对照组给予基础治疗,观察组在对照组基础上加用通窍活血汤治疗,两组治疗周期均为1个月。观察和比较两组治疗前后Lp-PLA2、NSE水平、糖脂代谢各指标以及复发情况的差异。结果:观察组总有效率为95.56%,显著高于对照组85.29%(P<0.05);治疗前两组Lp-PLA2、NES水平比较无统计学意义(P>0.05),治疗后观察组患者LpPLA2、NES水平分别为(41.03±7.79)μg/L、(17.59±6.68)μg/L均显著低于对照组(P<0.05);两组治疗后FPG、TC、TG、CHOL、LDL-C、HDL-C、INS水平较治疗前均有显著差异(P<0.05),治疗后观察组TC、Hb A1c、INS水平分别为(4.39±0.36)mmol/L、(5.03±0.47)%、(14.26±7.09)mmol/L显著较对照组低、LDL-C水平为(2.53±0.46)m U/L显著较对照组高(P<0.05)。观察组治疗1年后复发率为6.67%显著低于对照组23.53%(P<0.05)。结论:在临床常规治疗TIA患者基础上加用通窍活血汤能显著提高有效率,降低患者Lp-PLA2、NES水平,改善患者糖脂代谢,并减少复发,可推广使用。

Objective： To explore the effect of Tongqiao Huoxue Decoction on Lp-PLA2 level and glucolipid metabolism in patients with transient ischemic attack（ TIA）. Methods： 79 cases of patients with TIA who were admitted in the department of neurology in our hospital between December 2012 and December 2015 were selected as study subjects. The patients were divided into observation group（ 45cases） and controlled group（ 34cases） by the random number table method. The controlled group was given basic therapy. On the basis,the observation group was additionally treated with Tongqiao Huoxue Decoction. The cycle of treatment in the two groups was 1month. The levels of Lp-PLA2 and NSE,glucolipid metabolism indexes before and after treatment and recurrence were observed and compared between the two groups. Results： The total effective rate in the observation group（ 95. 56%） was significantly higher than that in the controlled group（ 85. 29%）（ P〈0. 05）; before treatment,the differences in levels of Lp-PLA2 and NES between the two groups were not statistically significant（ P〉0. 05）. After treatment,the levels of Lp-PLA2 and NES in the observation group [（ 41. 03 ± 7. 79） μg / L,（ 17. 59 ± 6. 68） μg / L] were significantly lower than those in the controlled group（ P〈0. 05）; after treatment,the levels of FPG,TC,TG,CHOL,LDL-C,HDL-C and INS in the two groups were significantly different from those before treatment（ P〈0. 05）. After treatment,the levels of TC,Hb A1 c and INS in the observation group [（ 4. 39 ± 0. 36） mmol / L,（ 5. 03 ± 0. 47） %,（ 14. 26 ± 7. 09） mmol / L]were significantly lower than those in the controlled group while LDL-C level [（ 2. 53 ± 0. 46） m U / L] was higher than that in the controlled group（ P〈0. 05）. The recurrence rate in the observation group after 1year of treatment（ 6. 67%） was significantly lower than that in the controlled group（ 23. 53%）（ P〈0. 05）. Conclusion： To apply Tongqiao Huoxue Decoction in the treatment of patients with TIA,based on clinical routine therapy can significantly improve the effective rate,reduce levels of Lp-PLA2 and NES,improve glucolipid metabolism and reduce the recurrence. It can be widely used.
