摘要
目的 观察丹参酮ⅡA磺酸钠联合拉米夫定对慢性乙型肝炎转化生长因子-β1肝功能的影响。方法 将十堰市太和医院感染科2014年4月～2015年7月收治的380例慢性乙型肝炎患者分为观察治疗组、丹参酮ⅡA磺酸钠组、拉米夫定组、丹参酮＋拉米夫定组,检测采用不同治疗方案治疗后四组患者转化生长因子-β1TGF-β1及血清肝纤维化血清学指标：IV型胶原（c IV）、层黏连蛋白（LN）、Ⅲ型前胶原（PC-Ⅲ）、透明质酸（HA）;肝微粒体细胞色素P450亚型酶（CYP2E1、CYPIA2、CYP3A4、CYP2D6、CYP2C9）活性;肝功能[胆汁酸（TBA）、总胆红素（STB）、谷丙转氨酶（ALT）、谷草转氨酶（AST）、谷氨酰转移酶（GGT）]。结果 丹参酮＋拉米夫定组TGF-β1、LN、PC-Ⅲ、c IV、HA、CYP2E1、CYP3A4酶活性、STB、TBA、ALT、GGT、AST均有不同程度降低,总有效率达95.79%,与其它三组比较,P〈0.05。结论 丹参酮ⅡA磺酸钠联合拉米夫定治疗慢性乙型肝炎疗效确切。
Objective To survey the effect of tanshinone ⅡA sulfonie acid sodium combined lamivudine on the transforming growth factor-β1 and liver function of chronic hepatitis B. Methods 380 chronic hepatitis B patients cured in infection branch of Shiyan taihe hospital during April 2014 to July 2015 were divided into four groups： observe treatment group, tanshinone ⅡA sulfonate group, lamivudine group, and tanshinone ＋ lamivudine group. Testing items of the patients before and after the treatment were transforming growth factor-β1 （TGF- β1） and serum liver fibrosis serological indexes： collagen type IV （cIV）, layer adhesion protein （LN）, type Ⅲ former collagen （PC-Ⅲ）, hyaluronic acid （HA）; liver particles somatic cell pigment P4s0 subtype enzyme activity （CYP2E1, CYPIA2 and CYP3A4, CYP2D6, CYP2C9）; Liver function [bile acid （TBA）, total bilirubin （STB）, alanine aminotransferase （ALT）, aspertate aminotransferase （AST）, glutamine transferase （GGT）]. Results The TGF-β1, LN, PC-Ⅲ, cIV, HA, CYP2E1, CYP3A4 enzyme activity, STB, TBA, ALT, GGT, AST of the tanshinone ＋ lamivudine group reduced by some degree, total effective rate was 95.79%, and comparing with the observe treatment group, tanshinone ⅡA sulfonate group and lamivudine group, P 〈 0.05, the differences were statistically significant. Conclusion The remedy of tanshinone ⅡA sulfonate combined lamivudine for chronic hepatitis B was curative.
