心脏手术中瑞芬太尼复合右美托咪定的麻醉效果及对血流动力学的影响

Effect of Anesthesia of Remifentanil Combined with Dexmedetomidine in Cardiac Surgery and Its Influence on Hemodynamics

目的分析心脏手术中瑞芬太尼复合右美托咪定的麻醉效果及对患者血流动力学的影响。方法选择医院2012年至2015年收治的先天性心脏病患者100例,随机分为试验组和对照组,各50例。试验组患者心脏手术采用瑞芬太尼复合右美托咪定联合麻醉,对照组患者心脏手术则采用单纯瑞芬太尼麻醉。比较两组患者插管及拔管期的应激反应,观察患者血流动力学改变情况。结果试验组患者插管及拔管期应激反应明显低于对照组(P<0.01);试验组患者血流动力学改变较对照组更小(P<0.01)。结论心脏手术中的麻醉应用瑞芬太尼复合右美托咪定的麻醉效果满意,且能降低患者的血流动力学改变,具有良好的应用价值。

Objective To analyze the effect of anesthesia of remifentanil combined with dexmedetomidine in cardiac surgery and its influence on hemodynamics. Methods 100 cases of hospitalized patients diagnosed as congenital heart disease in the hospital from 2012 to 2015 were selected and randomly divided into the experimental group and the control group,50 cases in each group. The ex- perimental group received remifentanil combined with dexmedetomidine for anesthesia in open cardiac surgery,while the control group received only remifentanil anesthesia method. The intubation and extubation stress responses in the two groups were compared, and the hemodynamic changes in the patients were observed. Results The stress response of intubation and extubation in the experimental group was significantly lower than that in the control group（P 〈 0. 01）. At the same time,the change of hemodynamics in the experimental group was smaller than that in control group（P 〈 0. 01）. Conclusion The application of remifentanil combined with dexmedetomidine in cardiac surgery has satisfactory anesthesia effect,can reduce the hemodynamic changes in patients with,and has good application val- ue in clinical surgery.
