脑微出血及其与抗栓治疗相关性研究进展 被引量:3

摘要 本文从脑微出血(cerebral microbleeds,CMBs)的流行病学、危险因素、病理机制以及诊断方法的研究进行综述,并对伴有CMBs的脑卒中(IS)患者的抗栓治疗的相关性研究进展进行介绍。目前对伴有CMBs的IS患者进行抗血小板治疗是否会增加脑出血(ICH)风险尚不确定,现有数据倾向于风险增加,但该风险弱于华法林治疗;尤其对于数量较多或(和)脑叶部位的CMBs,给予抗血小板治疗可能带来的ICH风险大于获益。对需溶栓治疗的伴有CMBs的急性IS患者,当CMBs数量>10个或(和)脑叶部位的CMBs时,应该慎用溶栓治疗。对需抗栓治疗的IS患者行头颅SWI检查,可明确CMBs存在的数目和部位,有利于拟定治疗方案。
