异氟醚后处理对大鼠心肌缺血再灌注损伤的影响 被引量:1

摘要 目的探讨异氟醚后处理对不同年龄大鼠心肌缺血再灌注损伤的影响。方法将成年健康雄性SD大鼠18只和老年健康雄性SD大鼠18只采用随机数字表法随机分为3组,对照组(C组)、缺血再灌注组(I/R组)、异氟醚后处理组(Iso PP组)。建立Langendorff离体心脏灌注模型,C组持续灌注130 min;I/R组平衡灌注10 min后缺血30 min再灌注90 min;异氟醚后处理组平衡灌注10 min缺血30 min,随后用含有1.5%异氟醚的灌注液灌注15 min,随后再灌注75 min。于平衡灌注末、再灌注30 min和90 min时记录HR、左室舒张末压(LVEDP)、左室发展压(LVDP)、左心室压力上升最大速率(+dp/dtmax)和左心室压力下降最大速率(-dp/dtmax)。灌注结束后,取心尖部心肌组织1 mm3电镜下观察线粒体结构并评分,剩余心脏组织采用TTC染色法测定心肌梗死面积。结果在成年鼠中,与I/R组相比,异氟醚后处理组心肌再灌注后的心功能指标改善,心肌梗死面积减小,线粒体损伤减轻(P<0.05);在老年鼠中,与I/R组相比,异氟醚后处理组心肌再灌注后的心功能指标、心肌梗死面积以及线粒体损伤程度均无明显改变(P>0.05)。结论异氟醚后处理减轻了成年老鼠的心肌再灌注损伤,而未能减轻老年鼠心肌的再灌注损伤。 Objective To investigate the isoflurane treatment for myocardial ischemia reper- fusion injury of rats at different ages. Methods A total of 18 healthy male SD rats and 18 healthy male SD rats were randomly divided into control group (group C ), ischemia-reperfusion (I/R group), isoflurane after treatment group (IsoPP group). Langendorff isolated heart perfusion model was established, group C was infused for 130 min, I/R group was re-perfused for 10 min and after is- chemia for 30 min, reperfusion was carried out for 90 min. Isoflurane treated group was re-perfused for 10 min and in ischemia for 30min, followed by containing 1.5% isoflurane perfusion for 15 min, and reperfusion for 75 min. At the end of balance perfusion, HR, left ventricular end-diastolic pres- sure ( LVEDP), left ventricular developed pressure ( LVDP), left ventricular maximum rate of pres- sure rise ( + dp/dt ) and left ventricular maximum rate of pressure drop ( - dp/dt ) were recor- ded after reperfusion for 30 min and 90 min. At the end of infusion, apical myocardial tissue 1 mm3 mitochondrial structure was observed by electron microscopy, infarct size was measured using the re- maining heart tissue TTC staining. Results In adult ,nice, compared with I/R group, cardiac func- tion indexes after sevoflurane treatment were improved after myocardial reperfusion, infarct size of mi- tochondrial damage mitigation was decreased ( P 〈 0.05 ) ; In older mice, compared with I/R group, the cardiac function after isoflurane myocardial reperfusion, the area and the degree of mitochondrial damage of myocardial infarction in isoflurane after treatment group were not significantly changed (P 〉 0.05). Conclusion Isoflurane after treatment can reduce myocardial reperfusion injury, and failure to reduce reperfusion injury of myocardium in aged rats.
