ICU护理风险干预在急性呼吸窘迫综合征患者中的应用效果

Application of risk nursing intervention in patients with acute respiratory distress syndrome in Intensive Care Unit

目的探讨ICU护理风险干预在急性呼吸窘迫综合征患者中的应用效果。方法选取在本院ICU接受治疗的急性呼吸窘迫综合征患者53例,随机分为护理风险干预组27例及常规护理组26例,所有患者均积极处理原发病、纠正低氧、机械通气、液体管理及营养支持,且2组患者分别接受相应护理干预方式,观察2组患者并发症发生情况,并在治疗结束时对不同护理干预方案进行量化考评,包括护理技能熟练度、护理质量控制评分、护理满意度评分及护理投诉发生率等。结果2组的误吸发生率均较低,而护理风险干预组的输液反应、静脉炎及呼吸机相关性肺炎的发生率显著低于常规护理组(P<0.05)。治疗结束后,护理风险干预组的护理技能熟练度、护理质量控制评分、护理满意度评分、护理投诉率均优于常规护理组,差异有统计学意义(P<0.05)。结论对ICU急性呼吸窘迫综合征患者实施护理风险干预,可明显降低并发症发生率,提高护理技能熟练程度、护理质量控制评分以及护理满意度,减少护理投诉事件,值得临床推广应用。

Objective To investigate application of risk nursing intervention in patients with acute respiratory distress syndrome in Intensive Care Unit. Methods A total of 53 patients with a- cute respiratory distress syndrome in ICU were randomly divided into risk nursing intervention group with 27 cases and usual care group with 26 cases. All patients were actively treated primary disease and given correct hypoxia, mechanical ventilation, fluid management and nutritional support. Then two groups were given corresponding nursing intervention. Complications were observed and, nursing interventions including nursing skills proficiency, care quality control rates, nursing satisfaction scores and the incidence of complaints were evaluated at the end of treatment. Results The incidence of aspiration was lower, and infusion reactions, phlebitis and ventilator associated pneumonia in risk nursing intervention group were significantly lower than the usual care group, and the differences were statistically significant （P 〈 0.05 ）. High skilled nursing, quality control scores, satisfaction score and complaints in the risk nursing intervention group were better than the usual care group, the differ- ence was statistically significant （ P 〈 0.05 ）. Conclusion Nursing intervention, compared to the traditional routine nursing intervention, can significantly reduce the incidence of complications of pa- tients with acute respiratory distress syndrome in ICU, improve nursing skills proficiency, quality of care, control scores of nurses and satisfaction, and reduce complaints.
