磁敏感加权成像对脑海绵状血管瘤的诊断价值 被引量:1

摘要 目的 探讨磁敏感加权成像(SwI)在诊断脑海绵状血管瘤中的影像学表现特点,以提高其诊断准确率.方法 回顾性分析70例经病理证实的脑海绵状血管瘤患者的MRI资料,将SWI与常规MR对比,评价SWI对脑海绵状血管瘤检出的优越性.结果 70例患者SWI均得到明确显示,单发40例(57.1%),多发30例(42.9%);SWI发现病灶436个,明显多于T1WI显示的98个(22.5%)和T2WI显示的132个(30.3%);典型表现为“爆米花样”或“桑葚状”混杂信号,边缘出现“铁环征”或呈典型“黑点征”.结论 SWI比常规MR对脑海绵状血管瘤的检出更为敏感,能检出更多微小病灶,常规MRI结合SWI能够对脑海绵状血管瘤行定性及定量诊断,有利于临床治疗及预后. Objective To evaluate the features of susceptibility weigeted image (SWI) in diagnosis of cerebral cavernous malformations(CCMs) and improve the diagnostic accuracy.Methods The MRI data of 70 patients with CCMs confirmed by pathology were retrospectively analyzed.The detection rate and the features of lesion were compared between SWI and conventional MRI,and the advantages of SWI in the diagnosis of CCMs were evaluated.Results SWI of 70 patients with CCMs were clearly displayed,40 cases(57.1%) were single and 30 cases(42.9%) were multiple.Four hundred and thirtysix lesions were detected by SWI,98 lesions were detected by T1WI,and 132 lesions were detected by T2 WI.The lesions showed popcorn or mulberry mixed signal on MRI,and hoop or dark spot like hypointense around the lesions.Conclusions SWI is more sensitive than conventional MR in the diagnosis of CCMs,and can detect more small lesions.Conventional MR combined with SWI can make qualitative and quantitative diagnosis of CCMs,which contributes to clinical treatment and prognosis.
