牙周内窥镜下龈下刮治和传统龈下刮治治疗牙周炎效果对比

Comparison of the effects of periodontal scaling under the periodontal endoscope and traditional periodontal scaling on periodontitis

目的 比较牙周内窥镜下龈下刮治和传统龈下刮治对牙周炎的治疗效果.方法 选取2014年5月至2015年5月48例中重度慢性牙周炎患者,根据治疗方案不同分为对照组与实验组,分别给予传统龈下刮治和牙周内窥镜下龈下刮治,观察两组疗效.结果 实验组在治疗后1、2d的VAS评分较对照组显著降低（P＜0.05）;治疗后第3个月,两组探诊深度（PD）、出血指数（BI）均有显著改善（P＜0.05）,附着丧失（AL）较治疗前稍有改善（P＞0.05）,两组治疗第3个月时PD、AL、BI比较差异未见统计学意义（P＞0.05）;治疗第3个月时,两组超敏C-反应蛋白（hs-CRP）、肿瘤坏死因子-α（TNF-α）、白细胞介素-17 （IL-17）较治疗前均显著改善（P＜0.05）,而实验组改善优于对照组（P＜0.05）.结论 牙周内窥镜下龈下刮治和传统方法均可显著改善牙周炎症,而前者对牙周炎的治疗效果更佳,且术后疼痛反应更轻,值得推广.

Objective To compare the curative effects of periodontal scaling under the periodontal endoscope and traditional periodontal scaling on periodontitis.Methods From May 2014 to May 2015,48 patients with moderate to severe chronic periodontitis were selected.According to the different treatment methods,they were divided into the control group and the experiment group,and the two groups took traditional periodontal scaling and periodontal therapy under the periodontal endoscope respectively,the curative effects of the two groups were observed.Results In the experiment group,1 day and 2 days after treatment,VAS score decreased significantly compared with the control group （P 〈0.05）.In the third months after treatment,probing depth （PD） and bleeding index （BI） of the two groups were improved significantly （P 〈 0.05）,attachment loss （AL） slightly improved compared with before treatment （P 〉 0.05）.Third months after treatment,PD,A1 and BI had no significant difference between the two groups （P 〉 0.05）.Three months after treatment,high-sensitivity C-reactive protein （hsCRP）,tumor necrosis factor alpha （TNF-α） and leukocyte interleukin 17 （IL-17） of the two groups were significantly improved （P 〈 0.05）,while the experiment group improved more significantly than control group （P 〈 0.05）.Conclusions Periodontal scaling under the periodontal endoscope and traditional methods can significantly improve periodontal inflammation,and the former has better effect on periodontitis,and postoperative pain response is more light,so it is worth promoting.
