1996-2015年国内外胃癌患者幽门螺杆菌阳性率变化特点的Meta分析

The changing characteristic of Helicobacter pylori positive rate of domestic and foreign patients with gastric cancer from 1996 to 2015: a Meta-analysis

摘要 目的评价近20年来我国胃癌患者幽门螺杆菌阳性率的变化特点,并与部分发达国家相比较,为临床防治提供依据。方法在Pubmed、Medline、Ovid、CNKI等数据库中检索相关文献,采用Meta分析对入选的42篇有关胃癌患者幽门螺杆菌阳性率的研究进行汇总、归纳和分析。其中入选的部分发达国家涵盖欧洲(德国、英国、瑞典等)、美洲(美国)、亚洲(日本),基本可以代表发达国家的整体水平。结果近20年来我国胃癌患者幽门螺杆菌阳性呈下降趋势(1996-2005年77%,2006-2015年61%),差异有统计学意义(P<0.05);部分发达国家胃癌患者中幽门螺杆菌阳性率未见明显变化(1996-2005年77%,2006-2015年76%),差异无统计学意义;1996-2005年,我国胃癌患者中幽门螺杆菌阳性率与部分发达国家差异无统计学意义;2006-2015年,我国胃癌患者幽门螺杆菌阳性率低于部分发达国家,差异有统计学意义(P<0.05)。结论近20年来我国胃癌患者幽门螺杆菌阳性率可能呈降低趋势,而部分发达国家可能无明显改变;1996-2005年,我国胃癌患者幽门螺杆菌阳性率与部分发达国家发病率相似,2006-2015年可能低于部分发达国家。 Objective To evaluate the changing characteristics of Helicobacter pylori positive rate of gastric cancer patients in the past 20 years.And to compare the Chinese rate with the rate of some developed countries during the same period,and provide the basis for clinical prevention and treatment.Methods Meta-analysis was used to summarizing the data extracted from 42 related studies,and the studies was retrieved in Pubmed, Medline, Ovid, CNKI.The developed countries which were chosen were located in Europe ( Germany,Britain,Sweden,etc) ,America( America) ,Asia( Japan) .They could represent the overall level of developed countries.Results In the past 20 years,there was a dramatic decline in the positive rate in China ( from 1996 to 2005:77%, from 2006 to 2015:61%) , and the change was statistically significant ( P 〈0.05).But the change in some developed countries was not obvious and has no statistical significance(from 1996 to 2005:77%,from 2006 to 2015:76%) .From 1996 to 2005,the rate had no difference between China and some developed countries.But from 2006 to 2015, the Chinese rate was lower than the rate of some developed countries ( P 〈0.05) .Conclusions The Helicobacter pylori positive rate in China probably declined in the past 20 years.But the rate of the developed countries probably had no difference .From 1996 to 2005, the Chinese rate probably had no difference with some developed countries.But from 2006 to 2015, the Chinese rate might be lower.
