早期康复护理对脊髓损伤神经源性膀胱患者膀胱功能及尿路感染的影响

Effect of early rehabilitation nursing on bladder function and urinary tract infections for neurogenic bladder patients with spinal cord injury

目的评价早期康复护理对脊髓损伤神经源性膀胱患者膀胱功能及尿路感染的影响。方法本研究收集脊髓损伤患者52例,随机分为对照组、观察组,各26例。对照组患者实施常规护理干预,观察组患者在对照组基础之上实施早期康复护理。对比2组患者膀胱功能以及尿路感染发生率差异。结果观察组患者尿管留置时间平均(45.3±2.8)d,残余尿量平均(45.1±1.9)m L,均显著低于对照组(P<0.05);膀胱容量平均(410.2±23.8)m L,最大排尿量平均(256.8±18.9)m L,均显著高于对照组(P<0.05);观察组患者尿路感染发生率为11.54%(3/26),对照组为45.31%(11/26),观察组显著低于对照组(P<0.05)。结论早期康复护理对脊髓损伤神经源性膀胱患者膀胱功能恢复有重要影响,并可有效降低尿路感染事件发生率,值得引起临床重视并推广实践。

Objective To evaluate of early rehabilitation nursing on bladder function and urinary tract infections for neurogenic bladder patients with spinal cord injury. Methods A total of 52 neurogenic bladder cases in our hospital were randomly divided into control group, the observation group with 26 cases implemented early rehabilitation nursing, and control group was given usual care. Bladder function and incidence of urinary tract infections were compared. Results Indwelling catheter time average was （45.3±2.8） d, the average amount of residual urine was （45.1±1.9） mL in the observation group, which was significantly lower than the control group （P〈0.05）; Average bladder capacity was （410.2 ± 23.8） mL, maximum voided volume average was （256.8±18.9） mL in the observation group, which were significantly higher than the control group （P〈0.05）. Urinary tract infection rate in observation group was 11.54% （3/26） in the control group, and was 45.31% （11/26） in the observation group, which was significantly lower than the control group （P〈0.05）. Conclusion Early rehabilitation nursing has great impact on bladder function of spinal cord injury patients, and can effectively reduce the incidence of urinary tract infection.
