优势模式在2型糖尿病患者社区个案管理的效果探讨

Effect of Advantage pattern in patients with type 2 diabetes on community case management

目的探究优势模式在2型糖尿病患者社区个案管理的效果。方法选择168例糖尿病患者将其分成2组,观察组实施社区个案管理优势模式,对照组行常规模式,对比2组患者干预前、干预后3个月与半年的糖化血红蛋白水平和自我管理水平。结果干预后,2组患者糖化血红蛋白情况无显著差异(P>0.05);与干预前相比,观察组患者干预3个月和半年后,相关指标分别下降了0.68%和1.21%。和对照组相比,观察组患者的自我管水平较好,(P<0.05)。结论实施社区个案化管理优质模式可全面改善Ⅱ型糖尿病者的代谢水平和自我管理能力。

Objective To explore effect of Advantage pattern in patients with type 2 diabetes on community case management. Methods A total of 168 diabetic patients were divided into two groups, observation group implemented community case management and the control group applied regular pattern, glycosylated hemoglobin level and management level before and three months and six months after the intervention were contrast. Results After intervention, there was no significant difference in glycosylated hemoglobin in two groups（P〉0.05）. Compared with intervention before, the indicators of patients in the observation group in three months and six months after the intervention dropped 0.68% and 1.21%. Compared with the control group, and self management of the observation group was better（P〈0.05）. Conclusion The implementation of community case management can improve the metabolism and self management ability for type Ⅱ diabetes patients.
