冠心病室性早搏病人血小板参数的相关变化及临床意义

Related Change and Its Clinical Significance of Platelet Parameters in Patients with Coronary Heart Disease and Premature Ventricular Premature

目的 探讨冠心病室性早搏病人血小板计数（PLT）、血小板比容（PCT）的相关变化及临床意义。方法 选取中国中医科学院广安门医院2009年1月—2015年6月心血管科住院的冠心病室性早搏病人125例,24h动态心电图（holter）记录室性早搏总数,分为Ⅰ组（〈1 000个）52例,Ⅱ组（1 000个-10 000个）53例,Ⅲ组（〉10 000个）20例,入院后收集病人一般情况及测定血液生化指标,比较3组间血小板参数水平。结果 PLT：Ⅲ组[（245.15±101.48）×10^9/L]〉Ⅱ组[（199.73±74.19）×10^9/L]〉Ⅰ组[（190.85±54.04）×10^9/L]组,Ⅲ组与Ⅰ组（P〈0.01）、Ⅱ组（P〈0.05）间差异有统计学意义。PCT：Ⅲ组（0.27±0.11）%〉Ⅱ组（0.21±0.08）%〉Ⅰ组（0.20±0.06）%,Ⅲ组与Ⅰ组（P〈0.01）、Ⅱ组（P〈0.05）间差异有统计学意义。相关性分析结果显示：室早总数与PLT（r=0.188,P〈0.05）、PCT（r=0.198,P〈0.05）呈正相关;室性早搏负荷与PCT（r=0.181,P〈0.05）呈正相关。结论 血小板计数、血小板比容的变化与冠心病室性早搏有相关性,对冠心病室性早搏的诊治及预防具有重要意义。

Objective To investigate the variation and clinical significance of platelet parameters（PLT,PCT）in patients with coronary heart disease（CHD）and premature ventricular contraction（PVC）.Methods One hundred and twenty-five hospitalized patients with CHD were grouped as group A（PVCs1 000）,group B（PVCs 1 000 to 10 000）,group C（PVCs10 000）,according to 24 h dynamic electrocardiogram monitoring.The general information and the data of the biochemistry of serum were collected,and the levels of platelet parameters were compared.Results PLT were 245.15±101.48 in group C,199.73±74.19 in group B,190.85±54.04 in group A.The difference was statistically significant between group C and group A or group B（P〈0.05）.PCT were 0.27±0.11 in group C,0.21±0.08 in group B,0.20±0.06 in group A.The difference was statistically significant between group C and group A or group B（P〈0.05）.Correlation analysis showed that there was a positive correlation between PVCs and PLT（r =0.188,P〈0.05）,PCT（r =0.198,P〈0.05）.Conclusion PLT and PCT are related to PVCs.Clinical monitoring of the changse of PLT and PCT is important for the diagnosis,treatment and prevention in patients with CHD and PVC.
