“考拉式体位”对剖宫产术后母乳喂养的影响

Influence of “koala-style” position on breast feeding in puerpera with caesarean section

[目的]探讨"考拉式体位"对剖宫产术后母乳喂养的影响。[方法]选取200例剖宫产产妇,随机分为观察组和对照组各100例。对照组采用传统坐位横抱式体位进行母乳喂养,观察组采用"考拉式体位"进行母乳喂养,比较两组产妇伤口疼痛视觉模拟评分(visual analogue scale,VAS)、乳汁淤积发生率、乳头皲裂发生率、泌乳量及母乳喂养率。[结果]产妇术后1d、3d、5d伤口VAS评分、母乳喂养率两组间比较差异均有统计学意义(P<0.05);产妇乳头皲裂发生率及乳汁淤积发生率两组间比较差异具有统计学意义(P<0.05),观察组均优于对照组。[结论]采用"考拉式体位"进行母乳喂养,可明显减轻产妇伤口疼痛,降低乳头皲裂和乳汁淤积发生率,提高泌乳量及母乳喂养率。

Objective：To probe into the influence of“koala style”position on breast feeding in puerpera with cae-sarean section.Methods：A total of 200 puerpera with caesarean section were randomly divided into control group and observation group,100 cases in each.The seat horizontal holding position was used in control group and the koala position was used in observation group.The visual analogue scale （VAS）score,cracked nipple rate,galactostasis rate,lactation volume and breast feeding rate were compared between both groups.Results：On the postoperative 1st day,3rd day and 5th day,the VAS score of observation group was respectively（4.3±0.8）, （3.2±0.7）and（2.8±0.6）,that in control group was respectively（5.7±1.0）,（4.4±0.5）and （3.4±0.5）,the difference showed statistically significant（P 〈0.05）.The cracked nipple rate and galactostasis rate of observa-tion group was respectively 5.0%（5/100）and 4.0%（4/100）,that in control group was respectively 26.0%（26/100）and 18.0%（18/100）,the difference showed statistically significant（P 〈0.05）.On postoperative 1st day, 3rd day and 5th day,the lactation volume of observation group was respectively （28.4±2.2）ml/d,（42.3±3.4） ml/d and （282.3±24.6）ml/d,that in control group was respectively（17.3 ±2.5）ml/d,（57.6 ±3.5）ml/d and （207.6±30.0）ml/d,the difference showed statistically significant（P 〈0.05）.On postoperative1st day,3rd day and 5th day,the breast feeding rate of observation group was respectively 75.0%（75/100）,80.0%（80/100）and 82.0%（82/100）,that in control group was respectively 62.0%（62/100）,67.0%（67/100）and 69.0%（69/100）, the difference showed statistically significant（P 〈0.05）.Conclusion koala position on breast feeding in puer-pera with caesarean section could effectively reduce wound pain,decrease the incidence of cracked nipple and galactostasis ,increase the lactation volume and breast feeding rate.
