慢性心力衰竭合并贫血的临床特点分析 被引量:2

摘要 目的:通过分析慢性心力衰竭合并贫血患者的临床特点,探讨心衰和贫血之间的联系。方法选取萍乡市人民医院住院确诊为慢性心力衰竭患者82例,依据患者有无贫血分为贫血组和无贫血组。收集2组患者临床资料,进行统计分析。结果慢性心力衰竭患者合并贫血在临床上具有较高的发生率,约为32.9%。2组患者比较发现,心衰合并贫血的患者,年龄偏大、BNP水平更高、肾功能趋向于恶化,且住院天数显著延长。对贫血原因进行的分析表明,缺铁性贫血仅占37.03%,慢性病性贫血可能是另一重要原因。对年龄、BNP、射血分数、肾功能、贫血等进行多因素分析表明,贫血是慢性心力衰竭的独立危险因素(OR=2.542,95%CI 1.213~5.322,P=0.016)。结论贫血是慢性心力衰竭的独立危险因素,心衰合并贫血的患者年龄偏大、BNP水平更高、肾功能趋向于恶化。 Objective This study analyzes the clinical characteristics of patients with chronic heart failure and anemia, aimed to explore the relationship between heart failure and anemia. Methods We selected, in a prospective and consecutive way, 82 patients who had been hospitalized for compensation of heart failure and anemia, according to the patients with or without anemia, we divided these patients into two groups:with and without anemia group. clinical characteristics of both groups were collected for statistical analysis. Results Chronic heart failure patients with anemia in clinic with high incidence of 32.9%. comparing Two groups of patients, we found that patients with heart failure combined anemia were older, with BNP level higher, tend to be deteriorated renal function, and extend hospitalization days signiifcantly. Analysis of anaemic reason shows that only 37.03%of the patients were iron deifciency anemia, chronic anemia may be another important reason. Anemia is the independent risk factors of chronic heart failure (OR=2.542, 95%CI 1.213 to 5.322, P=0.016). Conclusion The anemia is the independent risk factors of chronic heart failure, patients with heart failure combined anemia e were older, with BNP levels higher, kidney function tends to deteriorate.
