强直性脊柱炎伴发葡萄膜炎20例临床特征分析被引量：2

摘要目的:探讨强直性脊柱炎(AS)伴发葡萄膜炎的临床特征。方法:回顾分析20例AS伴发葡萄膜炎患者的临床资料。结果:AS发病年龄24.5±12.2岁;葡萄膜炎发病年龄28.5±13.1岁。其中12例患者先确诊为AS后就诊于眼科,8例患者以葡萄膜炎首诊于眼科后确诊为AS。其中单侧葡萄膜炎12例,双侧8例,15例有反复葡萄膜炎发作病史。化验检查HLA-B27阳性者19例,所有患者影像学检查均显示骶髂关节炎,髋关节受累3例,8例(40%)血沉升高,13例(65%)C反应蛋白升高。葡萄膜炎评分为5.8±1.8,强直性脊柱炎活动度评分ASDAS-CRP 1.71±0.91,ASDAS-ESR 1.73±0.81,葡萄膜炎评分与其无明显相关性。结论:AS伴发葡萄膜炎多累及青中年男性,易复发;根据典型的临床特征及影像学检查可明确诊断;葡萄膜炎严重程度与AS病情活动不一定相关。
