摘要
目的:探讨原发性高血压伴阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者心血管危险因素。方法:选取132例原发性高血压伴阻塞性睡眠呼吸暂停低通气综合征患者资料进行分析,采用自拟问卷调查表对患者一般情况进行统计、分析,统计心血管危险的单因素,并对单因素进行多因素Logistic分析,研究原发性高血压伴阻塞性睡眠呼吸暂停低通气综合征患者心血管危险因素。结果:从单因素分析可以看出原发性高血压伴阻塞性睡眠呼吸暂停低通气综合征心血管危险与年龄、性别、病程、空腹2 h血糖、尿酸高、肾小球滤过率偏低等因素差异不显著(P>0.05);原发性高血压伴阻塞性睡眠呼吸暂停低通气综合征心血管危险BMI、高血压分级、OSAHS分度、家族遗传病史以及高血脂关系密切(P<0.05);多因素Logistic分析,实验结果显示:BIM、家族遗传病史、OSAHS分度、高血压分级、高血脂等均为心血管病危险因素(P<0.05)。结论:原发性高血压伴阻塞性睡眠呼吸暂停低通气综合征患者发生心血管疾病危险因素相对较多,相关部门应采取有效措施改善患者危险因素,降低心血管疾病发生率。
Objective To investigate the primary hypertension with obstructive sleep apnea hypopnea syndrome cardiovascular risk factors. Method 132 cases of patient for treatment of essential hypertension associated with obstructive sleep apnea hypopnea syndrome were selected,the data of patients were analyzed,using our hospital from the proposed questionnaire for patients generally statistics analysis,statistics of single cardiovascular risk factor,and the multi- factor Logistic univariate analysis,essential hypertension with obstructive sleep apnea hypopnea syndrome cardiovascular risk factors. Results Univariate analysis can be seen in primary hypertension with obstructive sleep apnea hypopnea syndrome and cardiovascular risk age,gender,disease duration,2 h fasting glucose,uric acid,low glomerular filtration rate,etc. Factors were not significantly different( P〉0. 05); primary hypertension with obstructive sleep apnea hypopnea syndrome cardiovascular risk BMI,high blood pressure classification,OSAHS indexing,family genetic history and high cholesterol were closely related( P〈0. 05); Logistic multivariate analysis,results showed that: BIM,family genetic history,OSAHS indexing,classification of hypertension,high cholesterol and other risk factorswere incidence of cardiovascular risk( P〈0. 05). Conclusion Primary hypertension with obstructive sleep apnea syndrome ventilation occurs relatively more cardiovascular disease risk factors,the relevant departments should take effective measures to improve patient risk factors,reduce the incidence of cardiovascular disease.
