摘要
目的探讨高血压合并代谢紊乱的状况并分析其对心肾血管的影响。方法方便选取该院于2011年1月—2016年1月收治的1 200例高血压患者,根据不同的形式分成单纯高血压组、合并代谢综合征组、合并血脂紊乱组、合并腹型肥胖组、合并糖尿病组,行血管、心脏结构、功能等常规检查,分析患者MAU情况,对心肾血管进行评估。结果高血压合并腹型肥胖组的左室质量(LVH)和左室指数(LVMI)情况明显高于合并MS组、合并血脂紊乱组及合并糖尿病组(P<0.05);MAU阳性率,合并MS组检出率46.9%,合并糖尿病组44%,明显高于合并血脂紊乱组21.2%及合并腹型肥胖组20%,差异具有统计学意义(P<0.05);颈动脉斑块检出率,合并糖尿病组达57.3%,合并MS组达51.1%,合并血脂紊乱组及合并腹型肥胖组检出率分别为41.6%、37.1%,差异具有统计学意义(P<0.05)。结论临床医师在治疗高血压合并代谢紊乱时,应针对不同形式的代谢紊乱采用针对性的措施。
Objective To observe the condition of hypertension complicated with metabolic disorder and analyze its influence on the blood vessels of the heart and kidney. Methods Convenient according to the different forms, the 1 200 cases of hypertensive patients from January 2011 to January 2016 were divided into hypertension group, patients with the metabolic comprehensive syndrome group, complicating blood fat disorder group, combined with abdominal obesity group, diabetes mellitus group, routine examination of vascular, cardiac structure, function and so on. Analysis the Mau in patients with,carries on the appraisal to the cardiovascular and renal.Results Hypertension complicated with abdominal obesity group of left ventricular mass(LVH) and left ventricular mass index(LVMI) was significantly higher than that with MS group, combined with blood lipid disorder group and diabetes mellitus group(P〈0.05); positive rate of Mau, with MS group, the positive rate was 46.9%, with diabetes group 44%, significantly higher than and blood lipid disorder groups 21.2% and combined with abdominal obesity group 20%, the difference is statistically significant(P〈0.05); the detection rate of carotid artery plaque, with diabetes group of 57.3%, with MS group was 51.1%, with blood fat disorder group and combined with abdominal obesity group detection rate was 41.6% and 37.1% respectively, the difference was statistically significant(P〈0.05). Conclusion Clinicians in the treatment should be according to the different forms of metabolic disorder in the measures.
