摘要
目的探讨关节镜手术与保守方法治疗慢性膝关节骨性关节炎的临床疗效。方法整群选择该院骨科自2012年1月—2013年12月收治的114例慢性膝关节骨性关节炎患者为研究对象,并且按照随机数字表法分为对照组和观察组,每组57例。对照组患者接受保守治疗,观察组患者接受膝关节镜手术治疗。采用膝关节综合评分表和Lysholm评分系统评价治疗效果。结果治疗6个月后观察组患者优良率明显高于对照组患者,差异具有统计学意义(95.74%vs 72.34%,P<0.05)。对照组治疗前后Lysholm评分分别为[(43.56±5.01)vs(68.56±6.46)],观察组治疗前后Lysholm评分分别为[(43.25±4.28)vs(86.24±7.04)],治疗后两组Lysholm评分较前均明显升高(与同组治疗前相比,P<0.05),且观察组升高更为明显(与对照组治疗后相比,P<0.05)。结论膝关节镜手术治疗慢性膝关节骨性关节炎效果显著,对患者创伤小,有利于术后膝关节功能恢复,值得在临床中推广应用。
Objective To investigate the clinical effect of arthroscopic surgery and conservative treatment of chronic knee osteoarthritis. Methods Group selection 114 cases of chronic knee osteoarthritis treated in our hospital from January 2012 to December 2013 were divided into control group and observation group according to the random number table, 57 cases in each group. Patients in the control group received conservative treatment, the observation group were treated with arthroscopic surgery. The therapeutic effect was evaluated by using the comprehensive score of knee joint and the Lysholm scoring system. Results After 6 months treatment, the excellent rate of the observation group was significantly higher than that in the control group, the difference was statistically significant(95.74%vs 72.34%, P〈0.05).The Lysholm scores of the control group were respectively [(43.56±5.01)vs(68.56±6.46)],the Lysholm scores of the observation group were respectively[(43.25±4.28)vs(86.24±7.04)].After treatment, the Lysholm scores of the two groups were significantly higher than before treatment(compared with the same group before treatment, both P〈0.05), and the observation group increased more significantly compared with the control group(P〈0.05). Conclusion Knee arthroscopy in the treatment of chronic knee osteoarthritis has significant effect on patients with small trauma, it is beneficial to the recovery of knee joint function, it is worth to be popularized in clinical application.
