摘要
目的探讨在硅油眼硅油取出术中,使用微创23G可控制的压力抽吸硅油的方法对硅油眼手术的疗效。方法收集2013年2月-2015年10月在小榄医院住院80例硅油眼病例,使用23G注入及抽吸硅油功能的微创玻璃体切割仪,按照玻璃体切割常规三通道切口,做好灌注、照明及抽吸,使用抽吸硅油功能,根据手术的情况,抽吸压力可控制在理想的压力水平。在硅油抽吸的手术前段、中段及后段早期使用的抽吸压力控制在600—630mmHg,在手术的后段末只剩下一个大的硅油珠时,抽吸力控制在100—200mmHg之间。随着硅油珠逐渐吸除干净,压力逐步减少为零。对伴有黄斑或者后极部视网膜有增殖膜的患者同时行膜剥离手术,部分患者进行补充光凝或气体填充。结果所有80例手术患者手术全过程整个眼内压平稳,手术顺利。抽吸硅油时间约13。15rain,无并发症发生;随访3月,无视网膜脱离复发。结论微创23G可控性抽吸法硅油取出简单、安全、平稳、可行。
Objective To observe the clinical efficacy of controllable suction method for silicone oil removal with 23-Gange vitrectomy system. Methods All procedures were performed using the 23-gauge surgical vitrectomy system with the function of silicone oil suction. Beginning the surgery, 23-gauge microcannutas were placed using a trocar-cannula system. The microcannula in the superotemopral region was chosen for silicone oil removal. According to the operation, the suction pressure could be controlled at an ideal pressure level. Eighty patients (80 eyes) from Xiaolan people's hospital of Zhongshan fi'om February 2013 to October 2015 underwent silicone oil removal using the method mentioned above. The silicone oil was removed actively with suction pressure of a 600-630 mm Hg in the operation. At the end of surgery, when the remaining oil bubble was observed, the suction pressure was set at the level of 100-200mmHg, after removal of silicone oil, additional procedures, including endolaser, membrane peeling and gas filling were performed as needed. Results The intraocular pressure was stable in complete removal of the silicone oil. The duration of silicon oil removal was 13-15min, and the complication was not observed. During 3 months follow-up, retinal detachment did not occur. Conclusions The controllable suction method for silicone oil removal with 23-Gauge vitrectomy system was simple, safe, stable and feasible.
