摘要 目的 探讨磁共振扩散加权成像(diffusion-weighted imaging,DWI)技术对糖尿病患者肾脏受损情况的研究价值。方法 对68例2型糖尿病患者及14例正常志愿者行肾脏DWI检查,糖尿病患者根据尿A/C比值分为尿A/C比值正常糖尿病组、早期糖尿病肾病组(A/C比值30~300mg/g)、临床糖尿病肾病组(A/C〉300mg/g)。测量肾脏的表面弥散系数(ADC值)并进行分析。结果 临床糖尿病肾病患者较正常组志愿者的肾脏ADC值明显减低,有明显统计学差异(P=0.004)。结论 临床糖尿病肾病患者肾脏水分子弥散运动受限,DWI能够探测并反映糖尿病患者肾组织发生的异常改变。 Objective To investigate the possible damage to the kidneys and pathological changes of diabetic patients by MR diffusion weighted imaging (DWI) techniques. Methods DWI was performed in 68 diabetic patients with DN and 14 controls, and we collected data for image post-processing and analyzed the ADC values of cortex and medulla and compared between the two groups. According to the ratio of urinary albumin and creatinine (A/C ratio) , diabetics were divided into A/C ratio of normal dia- betic group (DM1 group), mieroalbuminuria diabetic group (DM2 group), macroalbuminuria in diabetic group (DM3 group). Results The ADC values of cortex and medulla DM3 group diabetics were than normal group of volunteers, which was signifi- cantly reduced, there were significant differences between the two ( P 〈 0.05). Conclusion DWI (ADC values) can detect and reflect the occurrence of renal tissue of diabetic patients who have abnormal pathology.
