超声心动图诊断右心衰竭的判别分析 被引量:11

摘要 目的分析超声心动图各参数对右心功能评价的效果,寻找最佳判别模型并探讨其在诊断右心衰竭中的价值。方法选取右心衰竭患者45例为病例组,无右心衰竭者36例为对照组,应用超声心动图测量并计算各参数:包括右室面积变化率(RVFAC)、三尖瓣环收缩期位移(TAPSE)、三尖瓣口舒张早期峰值流速(E)、三尖瓣环收缩期峰值速度(Sm)、舒张早期峰值速度(Em)、Tei指数、三尖瓣环等容收缩期加速度(IVA)及右室游离壁中间段收缩期应变率峰值(SRs)、舒张早期应变率峰值(SRe)。通过逐步判别分析筛选上述参数,建立判别模型,对该模型进行交叉验证,比较判别模型与常用单一参数的诊断效果。结果与对照组比较,病例组RVFAC、TAPSE、Sm、IVA、SRs及SRe均较对照组低,E/Em、Tei指数均较对照组高,差异有统计学意义(P<0.05)。通过逐步判别分析将贡献较大的RVFAC、TAPSE及IVA筛选出,建立判别模型:右心衰竭组F=0.246×FAC+1.356×TAPSE+2.746×IVA-15.587,非右心衰竭组F=0.435×FAC+2.093×TAPSE+4.634×IVA-39.706。留一交叉验证:右心衰竭组正确率为95.6%,非右心衰竭组正确率为97.2%,总正确率为96.2%,该结果较单一参数评价右心功能的效果理想。结论通过参数RVFAC、TAPSE及IVA建立的判别模型较单一超声参数的评价效果更理想,该模型在判断右心衰竭中具有重要临床意义。
