摘要 目的探讨CT引导下经皮穿刺植入放射性^(125)I粒子近距离内照射治疗晚期非小细胞肺癌的方法、疗效及并发症的处理。方法选择36例术前活检证实为非小细胞肺癌的患者,根据治疗计划系统(TPS)计算布源,于CT引导下经皮穿刺植入^(125)I粒子。术后即刻CT扫描验证,2~6个月复查CT观察粒子在瘤体内的分布、疗效及有无并发症,随访12个月。结果随诊CT复查,36例患者完全缓解(CR)12例;明显缓解(OR)19例;部分缓解(PR)5例;无效(P)0。1年生存率91.7%(33/36)。未见严重并发症和治疗相关的放射损伤。结论 CT导向下^(125)I粒子植入治疗非小细胞肺癌是一种安全、有效的方法,近期疗效肯定。 Objective: To discuss the clinical methods,effects and complications of CT guided radioactive seed ^125I implantation for non- small cell lung cancer( NSCLC). Methods: 36 NSCLC patients verified by puncture pathology,used interstitial implantation of ^125I seeds guided by CT with treatment planning system( TPS). Scanned by CT after implantation immediately,and scanned the tumor 2 to 6 months after the implantation in all cases to assess seed distribution,curative effects,and complications. Follow-up 12 months. Results: Among 36 follow-up cases,12 obtained CR,19 OR,5 PR,and0 P. 1 year survival rate was 91. 7%( 33 /36). No acute complications and late toxicity related to the implantation were observed. Conclusion: CT guided radioactive seed ^125I implantation is a safe and effective method,and has good short- term effects in the treatment of NSCLC.
