摘要
目的跟踪、分析1例Mondini畸形听障儿童植入人工耳蜗后的康复效果，为此类人工耳蜗植入儿童家长建立正确的期望值提供参考。方法用中国聋儿康复研究中心听觉语言能力评估工具及标准评估程序对1例mondini畸形听障儿童人工耳蜗植入术后不同阶段的听觉语言能力进行为期45个月的跟踪评估。评估数据采用单一被试实验方法进行检验。结果该儿童从开机后3个月开始，助听补偿效果为最适。听觉识别率基线期A（O～21个月）均数与处理期B（24～45个月）均数之间存在显著性差异（P〈0．05）；语言年龄基线期A（0～21个月）与处理期B（24～45个月）均数之间存在极显著性差异（P〈0．01）。结论①人工耳蜗植入手术使该名伴有Mondini畸形的双侧极重度感音性听障儿童重建听力，助听补偿效果明显；②开机2年内与2年后对比，前期听觉能力明显提高，后期发展缓慢甚至停滞不前。前期语言能力逐步提高，后期语言能力持续发展。
ObjectiveTo Follow up and analyze the rehabilitation effects of a child with Mondini dysplasia after cochlear implants. To provide right expectations reference for the parents of children with cochlear implants. Methods The child underwent cochlear implantation in the right ear and received full-time collective teaching and individual training at least half an hour a day afterwards. The auditory andspeech assessment tools and standard assessment procedures by China Rehabilitation and Research Center for Deaf Children were applied in the follow-up assessment at different postoperative intervals for 45 months.The evaluation data were tested by a single test method. Results From 3 months after switch-on, The results of hearing compensation quantity assessment were in the scope of ＂optimal＂ .There were significant differences in the auditory recognition rate and the language age between baseline period A（0-21months） and treatment period B（24-45months） （p〈0.05 and P〈0.01,respectively）.Conclusion Cochlear implantation reconstructed the hearing capability of the Mondini malformation patient with bilateral severe neuroauditory deafness, whose hearing compensation effects were noticeable after surgery.The comparison results between the data within 2 years and after 2 years since cochlear switch-on indicated the patient hearing capabilities had improved noticeably in the early period, but progressed slowly or stagnantly in the later period, while the language capability keeped on improving both in early and later periods.
