摘要
目的探讨喉癌患者术后发生咽瘘的原因及相关因素。方法回顾性分析我院收治的247例喉癌患者的临床资料，对基础疾病、手术方式、肿瘤分期等咽瘘的相关因素进行统计学分析。结果247例喉癌患者术后发生咽瘘26例，发生率为10．53％（26／247）。当患者合并糖尿病、全喉切除、同期行颈淋巴结清扫、中晚期肿瘤、手术时间〉5h，术后咽瘘发生率明显升高（P〈0．05）。结论喉癌术后发生咽瘘的相关危险因素有多种，控制基础疾病、加强围手术期管理、提高手术技能等可使咽瘘的发生率降低，提高喉癌患者的生存质量。
Objective To explore the causes and risk factors of pharyngocutaneous fistula after laryngeal cancer surgery. Methods We retrospectively analyzed the clinical data of 247 patients who were admitted into our hospital due to laryngeal cancer. Then, the risk factors associated with pharyngocutaneous fistula were evaluated. Results There were 26 patients with pharyngeal fistula after laryngeal cancer surgery, with an incidence of 10.53 % （26/247）. According to simple factor analysis, diabetes, laryngectomy, simultaneous neck dissection, advanced stage tumor, and more than 5 hours of operation time may play important roles in the pharyngocutaneous fistula after laryngeal cancer surgery （ P 〈 0.05 ）. Conclusions Postoperative pharyngocutaneous fistula is related to many factors. It is necessary to control the a- bove mentioned factors to reduce the rate of fistula and improve life quality.
