非功能性胰腺神经内分泌肿瘤超声表现与病理分级对照分析 被引量:8

摘要 目的探讨非功能性胰腺神经内分泌肿瘤(NF-PNETs)的超声表现,提高其术前诊断准确率。方法回顾性分析30例经穿刺或手术病理证实的NF-PNETs患者的超声表现,并与病理分级对照。结果 NF-PNETs发生于胰腺不同部位间的差异无统计学意义,而G1级肿瘤与G2、G3级肿瘤的大小比较差异均有统计学意义(P=0.00、0.01),G2级与G3级比较差异无统计学意义(P=0.62)。不同分级NF-PNETs的形态、内部回声比较差异均有统计学意义(P=0.001、0.013),而边界、回声及血流比较差异无统计学意义。肝脏、淋巴结转移及侵犯邻近组织血管均发生于G2、G3级NF-PNETs。结论超声检查显示的肿瘤形态、大小、内部回声及是否侵犯周围组织血管有助于NF-PNETs的术前分级判断。 Objective To investigate the ultrasonography findings of the non-functional pancreatic neuroendocrine tumors(NF-PNETs)and improve the preoperative diagnostic rate. Methods The ultrasonography findings of 30 patients with NF-PNETs confirmed by puncture or surgical pathology were retrospectively analyzed,and compared with pathologic grading.Results Among 30 patients,there were no statistically significant differences among NF-PNETs occurring in different parts of the pancreas,while G1 tumors had statistically significant differences in tumor size compared with G2 and G3 tumors(P=0.00,0.01),and there was no statistically significant difference between G2 and G3 tumors(P=0.62). There were statistically significant differences between morphology and internal echo in different grades of NF- PNETs(P = 0. 001,0. 013),while there were no statistically significant differences among boundary,echo and blood flow in different grades of NF-PNETs. All liver and lymph node metastases and the invasion of adjacent tissues and blood vessels occurred in patients with G2 and G3 NF-PNETs.Conclusion The ultrasonography can detect the size,morphology and internal echo of the tumor and the invasion of the surrounding tissues and blood vessels,which is conducive to be preoperative determination of the grades of the NF-PNETs.
