针灸联合中药熏蒸辨治颈肩腰腿痛的临床观察

Effects of Acupuncture Combined with Traditional Chinese Medicine Fumigation in Treatment of Pain in Neck Shoulder Low- Back and Leg

目的:观察和分析针灸联合中药熏蒸治疗颈肩腰腿痛的临床疗效。方法:选取100例颈肩腰腿痛患者作为研究对象,将其随机分为观察组和对照组,每组各50例,对照组患者在辨证论治的基础上给予针灸治疗,观察组患者在对照组疗法的基础上加用自拟中药汤剂熏蒸治疗。对两组患者的临床疗效及治疗前、后的证候评分、视觉模拟评分法(VAS)评分、日本骨科学会评分法(JOA)、Barthel指数评分、Fugl-Meyer功能活动评分进行观察和比较。结果:观察组患者的疗效和总有效率均显著优于对照组,两组之间的差异均有统计学意义(χ2=10.006,6.353,P<0.05);观察组患者治疗后的各项证候评分均低于对照组,两组之间的差异均有统计学意义(t=16.874、24.244、11.699,P<0.05);观察组患者治疗后的VAS评分显著低于对照组,JOA评分显著高于对照组,两组之间的差异均有统计学意义(t=17.638、9.698,P<0.05);观察组患者治疗后的Barthel指数评分和Fugl-Meyer评分显著高于对照组,两组之间的差异均有统计学意义(t=10.881、15.094,P<0.05)。结论:在辨证论治的基础上,应用针灸联合中药熏蒸治疗颈肩腰腿痛,能够更加显著地缓解患者的临床症状,改善其关节功能和肢体运动功能,提高患者的生活质量,具有较好的治疗效果。

Objective：To observe and analyze the therapeutic effects of acupuncture combined with traditional Chinese medi- cine fumigation in treatment of pain in neck shoulder low - back and leg. Methods ： One hundred cases of patients with pain in neck shoulder low - back and leg were selected as the research objects and randomly divided into the observation group and the control group,50 cases in each group. The patients in the control group were treated with acupuncture treatment on the basis of syndrome differentiation and those in the observation group combined self - made traditional Chinese medicine decoction fumiga- tion on the basis of the treatment in the control group. The clinical therapeutic effects and the symptom score, the Visual Analogue Scale （VAS） score, the Japanese Orthopedics Association （JOA） score, the Barthel index score, the Fugl -Meyer functional ac- tivities score before and after the treatment of the patients in the two groups were observed. Results ：The therapeutic effects and the overall clinical efficiency of the observation group were significantly better than those in the control group and the differences be- tween the two groups were statistically significant （ X^2 = 10. 006,6. 353 ,P 〈 0.05 ）. The symptom scores after the treatment of the patients in the observation group were significantly lower than those in the control group and the differences between the two groups were statistically significant （ t = 16. 874,24. 244,11. 699, P 〈 0.05 ）. The VAS score after the treatment of the patients in the observation was significantly lower than that in the control group and JOA score was significantly higher than that in the control group. The differences between the two groups were statistically significant （t = 17. 638,9.698, P 〈 O. 05）. The Barthel score and Fugl - Meyer score after the treatment of the patients in the observation group were significantly higher than those in the control group and the differences between the two groups were statistically significant （t = 10.881,15. 094, P 〈 0.05 ）. Conclusion ： On the basis of syndrome differentiation, the application of acupuncture combined with traditional Chinese medicine fumigation in the treatment of pain in neck shoulder low - back and leg can sign.ificantly relieve the clinical symptoms, improve joint function and limb activity function,improve the quality of life of patients and has good therapeutic effects.
