摘要
目的研究叉头蛋白(Fox M1)和下调因子(GDDR)在幽门螺杆菌相关胃癌中的表达。方法收集和选取2012年1月-2015年12月本院胃镜活检和手术后胃组织标本150份,其中包含正常胃黏膜组织30份,幽门螺杆菌相关慢性胃炎、肠化生、异物增生、胃癌组织各30份,利用TriZol提取试剂提取RNA并进行逆转录,采用实时荧光定量PCR和免疫组织化学法检测FoxM1基因及FoxM1和GDDR在各疾病阶段表达的情况。结果实时荧光定量PCR检测正常胃组织,慢性胃炎、肠化生、异物增生和胃癌组织FoxM1的mRNA表达水平分别为15.51±2.65、32.68±3.81、41.32±4.09、59.88±6.21和88.65±7.92,免疫组织化学法检测FoxM1相对表达量分别为7.62±1.72、23.21±3.11、33.67±3.91、51.35±5.01和79.56±7.05。GDDR表达评分分别为:正常组织>6分,由幽门螺杆菌引起的胃癌评分<4分,其中<2分(27份)占90%。在由正常组织受到幽门螺杆菌感染向胃癌转化过程中,GDDR表达呈下降趋势。讨论幽门螺杆菌是消化性溃疡和慢性胃炎的主要致病菌,也是引发胃癌的一个重要危险因素。FoxM1和GDDR在正常胃组织,幽门螺杆菌引发的胃炎以及胃癌中组织表达存在差异,检测FoxM1和GDDR可作为幽门螺杆菌引发胃癌的重要依据。
Objective To study the expression of FoxM1 and GDDR in gastric cancer associated with a Helicobacterpy loriinfection. Methods One hundred and fifty gastric tissue samples were collected from patients who underwent gastric biopsy or surgery from January 2012-December 2015.Of those samples,30 had normal gastric mucosa,30 had gastritis associated with an H.pylori infection,30 had intestinal metaplasia,30 had a gastric foreign body or hyperplasia,and30 had gastric cancer.RNA was extracted with TriZol and reversed transcribed.Expression of Fox M1 and GDDR in affected gastric tissue was determined with real-time fluorescence PCR and immunohistochemistry(IHC). Results Realtime fluorescence PCR indicated that the level of expression of FoxM1 mRNA was 5.51±2.65 in normal gastric tissue,32.68±3.81 in tissue with gastritis,41.32±4.09 in tissue with intestinal metaplasia,59.88±6.21 in tissue from a foreign body or tissue with hyperplasia,and 88.65±7.92 in gastric cancer tissue.IHC indicated that the relative level of FoxM1 expression was 7.62±1.72 in normal gastric tissue,23.21±3.11 in tissue with gastritis,33.67±3.91 in tissue with intestinal metaplasia,51.35±5.01 in tissue from a foreign body or tissue with hyperplasia,and 79.56±7.05 in gastric cancer tissue.GDDR expression was scored,with normal tissue having a score of over 6points and tissue with gastric cancer caused by H.pylori having a score of less than 4points.Ninety percent(27)of the 30 samples of gastric cancer tissue had a score of less than 2points.GDDR expression tended to decrease as H.pylori transformed normal tissue into cancer tissue. Conclusion Gastric carcinoma is the second most common malignancy in the digestive system.H.pylori is a pathogenic bacterium that causes peptic ulcers and chronic gastritis,but it is also a key risk factor for gastric cancer.Expression of FoxM1 and GDDR differed in normal gastric tissue and gastric cancer tissue,so detection of FoxM1 and GDDR is an important basis for prevention of gastric cancer associated with an H.pylori infection.
