某院2016年1—8月外科手术预防使用抗菌药物分析

Analysis of Preventive Use of Antimicrobial Drugs in the Surgical Operation in Some Hospital from January 2016 to August 2016

目的 了解某医院2016年1—8月外科手术预防性使用抗菌药物情况,评价用药合理性,为抗菌药物的临床应用管理提供数据支持。方法 回顾性调查某院2016年1月—8月的归档病例,每月对18个临床科室住院手术病历进行随机调查,填写患者基本情况和应用抗菌药物情况,对数据进行统计分析。结果 1 209份病历中,预防用药为801例,总围手术期预防使用抗菌药物比例66.25%,在初次用药方面,术前0.5-1 h未用术后用的病例占31.71%;存在使用不合理现象的681例中,预防时间长507例,术前0.5-1 h内未使用术后用抗菌药物的254例,选用抗菌药物种类不当的165例。结论 该院外科手术预防用抗菌药物存在预防用药时间长、用药时机不当、选药种类不当等问题,有待进一步加强管理。

Objective To know the condition of preventive use of antimicrobial drugs in the surgical operation in some hos-pital from January 2016 to August 2016 and evaluate the medication rationality thus providing data support for the clinical application management of antimicrobial drugs. Methods The archival cases in some hospital from January 2016 to August 2016 were retrospectively surveyed, the inpatient operative medical cases in 18 clinical departments were randomly sur-veyed every month, and the basic conditions of patients and application condition of antimicrobial drugs were filled, and the data were counted and analyzed. Results In the 1209 medical records, the preventive medication was in 801 cases, the pro-portion of preventive use of antimicrobial drugs accounted for 66.25% in the total perioperative period, in terms of the ini-tial medication, the medical cases without use in 0.5-1h before operation but with use after operation accounted for 31.71%, the unreasonable use was in 681 cases, the long preventive time was in 507 cases, 254 cases were without use of antimicro-bial drugs in 0.5-1h before operation but with use after operation, and 165 cases selected the inappropriate antimicrobial drug type. Conclusion The preventive use of antimicrobial drugs in the surgical survey in the hospital has the issues of long preventive medication time, inappropriate medication time and inappropriate drug-selection type, and the management of it needs to be further enhanced.
