临床药师干预小儿急性阑尾炎围手术期抗菌药应用的效果分析

Analysis of Effect of Clinical Pharmacist Intervention on the Application of Antibacterial Drugs of Children Acute Appendicitis in Perioperative Period

目的 评价临床药师干预小儿急性阑尾炎围手术期抗菌药应用的效果,为临床合理应用抗菌药物提供参考。方法回顾性分析2014年1-9月实施干预前的小儿急性阑尾炎手术病例182例（干预前组）,总结其抗菌药物应用中存在的主要问题并采取干预措施,选取2015年1—9月小儿急性阑尾炎手术病例193例（干预后组）进行对比分析,比较两组用药选择、给药剂量、给药频次、给药疗程、联合用药、更换药物等情况,评价围手术期抗菌药物应用的合理性及干预效果。结果 经临床药师干预,小儿急性阑尾炎围手术期抗菌药物选择合理率由66.48%升至93.26%（P〈0.05）,联合用药合理率由79.12%升至90.16%（P〈0.05）,给药疗程由平均10.28 d缩短至8.99 d（P〈0.05）,而干预前后给药剂量、给药频次的差异无统计学意义（P〉0.05）。结论 临床药师对小儿急性阑尾炎围手术期应用抗菌药物的干预是有效、可行的,能显著提高围手术期抗菌药物应用的合理性。

Objective To evaluate the application effect of clinical pharmacist intervention on the application of antibacteri-al drugs of children acute appendicitis in perioperative period. Methods 182 cases of children with acute appendicitis oper-ation before intervention from January 2014 to September 2014 were retrospectively analyzed, and major application issues of antibacterial drugs were summarized and intervention measures were adopted, 193 cases of children with acute appen-dicitis operation from January 2015 to September 2015 were selected for comparison and analysis, and the medication choice, administration dose, administration frequency, administration treatment course, combined medication and drug changes were compared between the two groups, and the application rationality of antibacterial drugs in perioperative period and intervention effect were evaluated. Results After the intervention of clinical pharmacists, the selection rational rate of antibacterial drugs of children acute appendicitis in perioperative period increased from 66.48% to 93.26%, P〈0.05, the ra-tional rate of combined medication increased from79.12% to 90.16%, P〈0.05, the administration treatment course decreased from 10.28d to 8.99d, P〈0.05, but the differences in the administration dose and administration frequency before and after intervention had no statistical significance, P〉0.05. Conclusion The intervention of clinical pharmacists on the application of antibacterial drugs of children acute appendicitis in perioperative period is effective and feasible, which can obviously improve the rationality of application of antibacterial drugs in perioperative period.
