康复新喷剂对支气管插管术后咽喉并发症的影响

Effects of Kangfuxin spray on throat complications after bronchial intubation

目的探讨康复新喷剂对支气管插管术后咽喉不适的影响。方法选择拟在全麻下行支气管插管的胸外科手术患者80例，年龄16～75岁，随机分为两组，每组40例。实验组于支气管插管前给予咽喉部喷洒康复新，并予康复新合并石蜡油进行气管插管的润滑，拔管后再次予以康复新喷剂行咽喉部喷洒，复苏后喷洒以及术后安返病房后以q4h继续喷洒用药。对照组仅予石蜡油润滑支气管导管，不做其他处理。全麻诱导3rain后经口插入Robertshaw双腔支气管导管。插管成功后将气管导管连接麻醉机给予间歇正压通气。术中采用静吸复合麻醉维持麻醉深度。观察两组患者术后咽喉并发症（声音嘶哑、吞咽痛、异物感、干燥感）的发生率及其严重程度。结果两组患者除干燥感外其他咽喉并发症的发生率、严重程度差异有统计学意义（P〈0．05），与对照组相比，实验组咽喉并发症的发生率、严重程度明显降低。结论康复新喷剂能有效降低全麻手术下支气管插管患者术后咽喉并发症及严重程度。

Objective To explore the effects of Kangfuxin spray on throat discomfort after bronchial intubation. Methods 80 patients with the age of 16-75 years old receiving thoracic surgical procedures who were proposed to be given bronchial intubation under general anesthesia were randomly assigned to two groups, with 40 patients in each group. The experiment group was given Kangfuxin spray at the throat before bronchial intubation, and was given Kangfuxin combined with paraffin oil for lubrication of intubation. They were further given Kangfuxin spray at the throat after extubation and during resuscitation. Spray was further given at q4h after patients＇ returning to the ward. The control group was only given paraffin oil for lubrication of bronchial catheter without other management. Robertshaw dual cavity bronchial catheter was intubated orally 3 rain after genera/anesthesia. After intubation, the bronchial catheter was connected to the anaesthesia machine for intermittent positive pressure ventilation. Intravenous inhalational anesthesia was applied during surgery to maintain anesthetic depth. Incidence rate of throat complications（hoarseness, swallowing pain, foreign body sensation and dry sensation） and the severity degree were observed after the surgery in the observation group. Results The differences of the incidence rate of throat complications and severity degree except dry sense between the two groups of patients were significant （P〈0.05）. Compared with the control group, the incidence rate of throat complications and severity degree in the experiment group were significantly reduced. Conclusion Kangfuxin spray is able to effectively reduce postoperative throat complications and severity degree in the patients receiving bronchial intubation under general anesthesia.
