24例双侧丘脑对称性病变患者MR特点分析

MR Characters of Bilateral Thalamic Lesions

目的:探讨双侧丘脑对称性病变患者的MR及临床表现特点。方法:回顾性分析24例双侧丘脑对称性病变患者的临床表现及MR特点。结果:肿瘤性病变5例,病毒性脑炎7例,代谢性病变肝豆状核变性4例,急性动脉性脑梗死5例,脑深静脉血栓形成伴双侧丘脑静脉性梗死3例。MRS有助于鉴别肿瘤性和非肿瘤性病变;急性脑梗死在DWI上弥散受限;ESWAN对顺磁性物质敏感;MRV可直观显示脑静脉。结论:双侧丘脑对称性改变可见于多种疾病,其常规MR表现类似,结合其临床表现及功能MR,有利于正确诊断及临床治疗方案的制定。

Objective： To study the MR imaging and clinical characters of patients with bilateral thalamic lesions. Methods： Clinical and MR characters of 24 patients with bilateral thalamic lesions were analyzed. Results： There were 5 cases of bilateral thalamic glioma, 7 cases of viral encephalitis, 4 cases of Wilson＇ s disease, 5 cases of acute infarction, 3 cases of deep cerebral venous thrombosis. MRS was helpful to differentiate tumor from non-tumor lesions. Acute infarction showed diffusion limited on DWI. ESWAN was sensitive to paramagnetic materials. Cerebral venous sinus could be shown on MRV imaging. Conclusions： Many kinds of diseases showed similar bilateral thalamic lesions on MR imaging. The clinical characters and functional MR imaging help to diagnosis and differential diagnosis and treatment projects.
