基于临床需求的针对性临床药学工作模式探索

Establishing the target clinical pharmacy practice pattern based on the clinical needs

建立一种高效的、有针对性的临床药学工作模式,提高临床药师的工作效率和更好的发挥临床药师的作用。方法以北京大学第三医院肾内科作为临床药师工作科室,首先调查肾内科医护人员对临床药学工作的具体需求,然后基于这些具体需求,有针对性的确定工作内容,建立一种高效的临床药学工作模式。结果 100%的调查对象认为需要临床药师提供药物相关信息,但医师和护士对于临床药师的工作需求有所不同。针对不同层次的需求,开展了提供特定的药学信息、基于药代动力学/药效学的个体化给药方案优化、治疗药物监测结果解释、复杂用药病例会诊、以及针对特定患者或者特定药物的患者教育等内容的临床药学工作。该工作模式获得临床医生和护士的认可。结论在临床药师相对短缺的情况下,建立针对临床需求的临床药学专业工作模式有助于提高临床药师的工作效率,促进患者合理用药,提高治疗效果。

Objective To establish an efficient,targeted clinical pharmacy practice pattern to improve the work efficiency of clinical pharmacists.Methods Carry out this investigation in nephrology department of Peking university third hospital. First,investigate the specific needs of clinical pharmacy from doctors and nurses in nephrology department,and then determine the specific clinical pharmacy work contents and duties based on the results of investigation. Finally,establish an efficient clinical pharmacy practice pattern. Results 100% of respondents believe that medication information provided by clinical pharmacists is useful.However,the requirements are different between doctors and nurses. For different demands,we provide specific medication information,optimize individual administration based on pharmacokinetics and pharmacodynamics,interpretion of therapuetic drug monitoring results,and giving consultation of medication to patients. Doctors and nurses appraise this practice pattern. Conclusion In the situation of relative shortage of clinical pharmacists,it is important to establishing the target clinical pharmacy practice pattern based on the clinical needs,since it’s helpful to improve the work efficiency as well as safety for patients during using medication.
