2015年医院门诊中药饮片处方不合理用药分析

Analysis on Irrational Drug Use of TCM Decoction Piece Prescription in the Hospital in 2015

目的分析门诊中药饮片不合理处方存在的问题,推动临床合理使用中药饮片。方法从2015年医院门诊中药饮片处方中随机抽取1 000张,将不合理处方分为不规范处方、用药不适宜处方及超常处方。采用列表统计的方法,找出医院门诊中药处方中存在的问题,并进行分析和干预。结果不规范处方占抽查处方的32.30%,不适宜处方占2.00%,超常处方占0.20%,用药味数32味以上的处方占2.10%,超剂量使用有毒性中药饮片的处方占9.30%。结论医院门诊中药饮片处方用药基本合理,但也存在一定的不合理用药情况,医生、药师应共同协作,加强处方的监督检查,提高处方质量,促进临床合理使用中药饮片。

Objective To analyze the quality of outpatient prescription of TCM decoction pieces and the condition of rational drug use, to promote rational drug use of TCM decoction pieces. Methods 1 000 pieces of outpatient prescription of TCM decoction pieces in the hospital in 2015 were randomly selected and the irrational prescriptions were divided into irregular prescriptions, inappropriate prescrip- tions and off-label prescriptions. The method of list statistics was used to find out the problems in out- patient prescription of TCM decoction pieces. Analysis and intervention were given. Results Irregular prescriptions accounted for 32. 30% of the prescription selected, inappropriate prescriptions accounted for 2.00%, off- label prescription accounted for 0.20%, prescriptions using more than 32 kinds of drugs accounted for 2. 10%, prescription of ultra dose of toxic herbal decoction pieces accounted for 9. 30%. Conclusion The outpatient prescription of TCM decoction pieces in the hospital is basically rational, but there are still some irrational drug uses. Doctors and phar- macists should collaborate, strengthen the supervision and check of prescription and improve the quality of prescription, to promote the rational use of TCM decoction pieces.
