355例骨科Ⅰ类切口手术围术期预防性应用抗菌药物的调查与分析

Investigation and Analysis on Application of Prophylactic Antibiotics in 355 Orthopedic Patients with Type Ⅰ Incision

目的:了解解放军第535医院(以下简称"我院")骨科Ⅰ类切口手术围术期预防性应用抗菌药物情况,为加强抗菌药物的合理应用提供参考。方法:随机抽取我院2015年1—6月355例骨科Ⅰ类切口手术患者的病历,对Ⅰ类切口手术围术期预防性应用抗菌药物的给药时间、用药疗程、预防性用药率等进行统计分析。结果:355例骨科Ⅰ类切口手术中,227例预防性应用抗菌药物,抗菌药物预防性使用率为63.94%,符合我院骨科相关拟订值;222例术前30 min^2 h给药,5例术后预防性应用抗菌药物,预防性应用抗菌药物的给药时间合理率为97.80%;预防性应用的抗菌药物主要为头孢替唑(160例,占70.04%),其次为头孢呋辛(49例,占21.59%);平均用药疗程为4.3 d,超过我院骨科相关拟订值(1.3 d)。结论:骨科Ⅰ类切口手术围术期预防性性应用抗菌药物还存在不合理现象,有待继续加强培训和管理。

To investigate the application status of prophylactic antibiotics in typeⅠincision in No.535 Hospital of PLA＆amp;nbsp;（hereinafter referred to as“our hospital”）, so as to provide reference for the rational application of antibiotics.METHODS：355 orthopedic patients with typeⅠincision admitted into our hospital from Jan.to Jun.2015 were randomly extracted. Statistical analysis was conducted on the application status of prophylactic antibiotics in terms of administration time, treatment course and prophylactic application rate, etc..RESULTS： Of the 355 cases, 227 cases received antibiotics directly before surgery, the prophylactic application rate was 63.94%, which was accord with the standard in orthopedic department.There were 222 cases with prophylactic antibiotics before surgery of 30 min-2 h, 5 cases with postoperative antibiotics.The rational rate of administration time of prophylactic antibiotics was 97.80%.The main prophylactic antibiotics were ceftezole（160 cases, 70.04%）, followed by cefuroxime（49 cases, 21.59%）.The average medication time was 4.3 d, higher than that of the standard in orthopedic department（1.3 d）.CONCLUSIONS：There are still some irrational phenomenon in orthopedic department of our hospital, which needs the further strength in training and management.
