腹部超声对炎症性肠炎早期诊断及活动期、缓解期评估的临床价值

Clinical value of abdominal ultrasound in the early diagnosis and evaluation of active stage and remission stage of inflammatory bowel disease

目的研究腹部超声对炎症性肠病（IBD）早期诊断及活动期、缓解期评估的临床价值。方法回顾性分析2014年2月至2015年8月我院90例IBD患者（观察组）临床病历资料,其中克罗恩病（CD）42例,活动期20例,缓解期22例;溃疡性结肠炎（UC）48例,活动期23例,缓解期25例。选择同一时期来我院体检健康者70例为对照组。采用腹部超声检查观察所有研究对象肠壁厚度、血流状态及肠壁内动脉阻力指数（RI）,分析腹部超声检查在IBD诊断及判断疾病活动性中的应用价值。结果观察组肠壁厚度为（4.21±1.36）mm,显著高于对照组,RI为（0.63±0.05）,显著低于对照组,差异均有统计学意义（P〈0.05）,两组间患者血流状态分级比较,差异有统计学意义（P〈0.05）。UC活动期和CD活动期肠壁厚度显著高于缓解期,RI显著低于缓解期,差异均有统计学意义（P〈0.05）。UC和CD活动期0-Ⅰ级血流状态例数显著低于缓解期,差异有统计学意义（P〈0.05）。结论腹部超声检查在IBD早期诊断和活动性评估中具有较高的应用价值,值得临床推广。

Objective To study the clinical value of abdominal ultrasound in the early diagnosis and evaluation of active stage and remission stage of inflammatory bowel disease（IBD）. Methods Clinical data of 90 patients with IBD（the observation group） in our hospital from February 2014 to August 2015 were analysed retrospectively,among them, there were 42 cases in Crohns disease（CD）, 20 cases in active stage and 22 cases in remission stage;48 cases in ulcerative colitis（UC）, 23 cases in active stage and 25 cases in remission stage. 70 healthy examinees in our hospital in the same period were selected as the control group. Abdominal ultrasound examination was used to observe the intestine thickness, blood flow state and the intestine internal artery resistance index（RI） in all subjects, application value of abdominal ultrasound examination in diagnosis of IBD and disease activity were analyzed. Results Intestine thickness of the observation group was（4.21 ±1.36）mm, which was significantly higher than the control group, RI was（0.63±0.05）, which was significantly lower than the control group（all P〈0.05）, there was a significant difference in patients＇ blood flow state grading in the two groups（P 0.05）. Intestine thickness during the period of UC active stage and CD active stage were significantly higher than the remission stage, RI was significantly lower than the remission stage（P〈0.05）. 0-Ⅰgrade blood flow state cases in the UC and CD active stage were significantly lower than the remission stage（P〈0.05）. Conclusion Abdominal ultrasound examination has a high application value in the early diagnosis and activity evaluation of IBD, worth clinical promotion.
