个案管理在早产儿延续护理中的应用

The use of case management in transitional care for preterm infants

目的探讨个案管理家庭延续护理模式对早产儿生长发育及神经行为的影响。方法将92例早产儿按住院时间段分为对照组45例、观察组47例。对照组实施早产儿常规护理及出院电话随访,观察组实施个案管理家庭延续护理至早产儿1周岁。结果观察组早产儿6个月、12个月身高、体质量,40周矫正胎龄神经行为测定得分及复诊率显著高于对照组(均P<0.05);出院后早产儿护理问题及再住院率显著低于对照组(均P<0.05)。结论对早产儿实施个案管理家庭延续护理,有助于早产儿生长发育,降低再入院率。

Objective To explore the effectiveness of transitional care using case management on growth and neurobehavioral development of preterm infants. Methods Ninety two preterm infants were randomly divided into a control group of 45 and an observation group of 4.7 according to their admission time. The control group was given routine care and telephone follow-ups after discharge, while the observation group received transitional care using the case management model until the preterm infants became 1 year old. Results Height and weight at 6 and 12 months, neonatal behavioral neurological assessment （NBNA） score at 40 weeks corrected gestational age, as well as revisiting rate as required, in the observation group were significantly higher than the control group （P〈0.05 for all）. The incidence rates of problems in family care and readmission rate were significantly lower in the observation group than the control group （P〈0. 05 for all）. Conclusion Application of case management in transitional care contributes to growth and neurobehavioral development of preterm infants, and decreases readmission rate.
