血清CRP水平测定在急性胰腺炎诊断中的临床价值

Clinical value of serum CRP level in diagnosis of acute pancreatitis

目的探讨血清CRP的检测在急性胰腺炎诊断的临床价值。方法采用免疫比浊法对2013年6月~2016年6月入院就诊资料完整的62例急性胰腺炎患者和45例正常人血清中CRP含量进行检测,同时对不同分型胰腺炎患者以及入院当天、3 d、5 d CRP水平进行检测比较分析。结果急性胰腺炎组CRP水平63.57±58.73 mg/L高于健康对照组6.83±5.14mg/L,组间比较存在差异性（P〈0.05）;重型胰腺炎组CR P水平108.6±24.5 mg/L显著高于轻型胰腺炎组48.21±13.73 mg/L,两组间存在差异性（P〈0.05）;入院当天,3 d,5 d测量重型胰腺炎组的CRP分别为108.62±24.51 mg/L、163.24±23.57 mg/L和94.87±31.43 mg/L,高于轻型胰腺炎组48.21±13.73 mg/L、62.64±23.56 mg/L和31.26±12.14 mg/L。两组间CR P水平差异性显著（P〈0.01）。入院3天两组的CRP水平达到最高,重型胰腺炎组下降较缓慢。结论 CRP可以作为急性胰腺炎患者病情评估的重要参考指标,具有重要的临床诊断价值。

Objective To investigate the serum CRP and its clinical value in diagnosis of acute pancreatitis. Methods Immune turbidimetric method, Serum CRP in 62 patients with acute pancreatitis from January 2013 to January 2016 at our hospital and 45 cases healthy control group were detected. At the same time, the CRP levels in patients were detected and compared among different types of panereatitis and different days including the day of admission, three days and five days after admission. Results The CRP level of acute panereatitis group was 63.57 ± 58.73 mg/L which was higher than that in healthy control group which was 6.83 ± 5.14mg/L, the difference between two groups was significant （p〈 0.05）; CRP level at the SAP, was 108.6 ± 24.5 mg/L, which was significantly higher than that at MAP ,which was 48.21 ± 13.73 mg/L. There were differences between two groups （p 〈 0.05）; CRP values of SAP on the day of admission, 3 day and 5 days were 108.62 ± 24.51 mg/L, 163.24 ± 23.57 , and 94.87 ± 31.43 mg/L respectively, which were higher than those of MAP, which were 48.21 ± 13.73 mg/L, 62.64 ± 23.56 mg/L and 31.26 ±12.14 mg/L. And the difference was significant between two groups （p〈 0.01）. Three days after being in hospital, CRP in the two groups reached the highest level and SAP decreased more slowly. Conclusion CRP can be used as an important reference index to evaluate on AP patients, it has an important clinical diagnostic value.
