摘要
目的探讨第二人体表皮生长因子受体(C-erbB-2)、雌激素受体(ER)、孕激素受体(PR)在子宫内膜息肉癌变组织中的表达意义及与微血管密度(MVD)的关系。方法应用免疫组化SP法检测C-erbB-2、ER和PR在50例正常子宫内膜、50例子宫内膜息肉及50例子宫内膜息肉癌变组织中的表达水平,分析其临床意义及相关性,及检测C-erbB-2与微血管特异性标记物CD34的表达,分析CerbB-2表达与MVD的关系。结果 C-erbB-2、ER、PR在子宫内膜息肉癌变组阳性率分别为62.00%、96.00%、98.00%,明显高于子宫内膜息肉及正常子宫内膜组织26.00%/8.00%、84.00%/76.00%、68.00%/64.00%,差异有统计学意义(P<0.05)。子宫内膜息肉癌变组织中的MVD显著高于正常子宫内膜组织(P<0.0001),子宫内膜息肉组织MVD显著高于正常子宫内膜组织(P<0.0001)。C-erbB-2与ER的表达呈负相关(r=-0.247,P<0.05),ER与PR的表达呈正相关(r=0.756,P<0.05)。CerbB-2表达与MVD呈显著正相关(r=0.476,P<0.05)。结论在子宫内膜癌中,C-erbB-2、ER及PR联合检测可作为早期诊断、指导内分泌治疗及判断预后的重要指标,且C-erbB-2在子宫内膜息肉癌变组织中高表达,可能参与了肿瘤血管的生成。
Objective To explore the expression of human epidermal - growth factor receptor 2 (C - erbB - 2) , estrogen receptor (ER) and progesterone receptor (PR) in malignancy of endometrial polyps and the relationship with angiogenesis. Methods Immunohistochemical analyses of C - erbB - 2, ER and PR were performed in 50 cases of malignant of endometrial polyps, 50 cases of endometrial polyps and 50 cases of normal endometrium. The correlations among the expression of Cerb - B - 2 and the CD34 - stained microvessel density (MVD) were analyzed. Results The positive expression of C - erbB - 2, ER and PR in normal endometrium, endometrial polyps and endometrial carcinoma had significant difference among them ( P 〈0.05) . The MVD had significant differences between endometrial polyps and normal endometrium ( P 〈0.0001). At the same time, the MVD had significant differences between normal endometrium, endometrial polyps ( P 〈0.0001). C - erbB - 2 expression was negatively correlated with ER in endometrial carcinoma ( r = -0.247, P 〈 0 .0 5 ) . ER expression was positively correlated with PR in endometrial carcinoma (r =0.756,P 〈0.05). A significant positive correlation between MVD and the expression of C - erbB - 2 was observed by Spearman rank correlation test ( r = 0.476 , P 〈0. 05). Conclusion The abnormal expression of C - erbB - 2 , ER and PR plays an important role in the occurrence and development of endometrial carcinomas. Both of them may be used as the important index of early diagnosis, guiding endocrine therapy and judgment of the prognosis of endometrial carcinoma, and over expression of Gerb - B - 2 in malignant of endometrial polyps may be involved in the angiogenesis and progesterone resistance.
