探讨新生儿重症高胆红素血症换血治疗的临床效果 被引量:1

摘要 目的研究换血疗法对重症高胆红素血症新生患儿的临床治疗效果。方法随机选择该院2011年1月—2015年6月期间收治的重症高胆红素血症新生患儿60例为调查对象,对患儿进行换血治疗,观察换血治疗前后患儿的血液指标变化情况。结果治疗前,新生患儿TBIL为(428±16.6)μmol/L,DBIL为(37.2±16.6)μmol/L,IBIL为(388.9±20.3)μmol/L;治疗后,新生患儿的TBIL为(189.3±12.2)μmol/L,DBIL为(27.7±11.3)μmol/L,IBIL为(178.8±14.6)μmol/L,组间差异有统计学意义(P<0.05);治疗前,新生患儿的K+(3.8±0.6)mmol/L、Na^+(136.8±4.2)mmol/L、Ca^(2+)(2.2±0.39)mmol/L;治疗后新生患儿的K+(3.6±0.9)mmol/L、Na+(139.8±3.6)mmol/L、Ca^(2+)(2.3±0.26)mmol/L;组间差异无统计学意义(P>0.05)。新生患儿凝血功能以及血液中的钾离子、钙离子和钠离子水平差异无统计学意义(P>0.05)。结论对重症高胆红素血症新生患儿实施换血疗法,不仅能够有效帮助其降低血液中的胆红素水平血液中的钾离子、钙离子和钠离子水平,还能够帮助新生患儿维持电解质平衡,对新生患儿的凝血功能以及体征无影响,安全性较高,值得在临床上推广和使用。 Objective To study the exchange transfusion of high blood bilirubin newborn children with severe clinical therapeutic effect. Methods Random selection in our hospital during January 2011 to June 2015 children with severe high blood bilirubin freshmen 60 patients treated as investigation object, and the treatment of children with change, observe change of changes in children with blood indexes before and after treatment. Results Before treatment, TBIL for newborn children(428 ± 16.6) μmol/L, DBIL(37.2±16.6) μmol/L, IBIL(388.9±20.3) μmol/L; after treatment, TBIL for newborn children(189.3±12.2) μmol/L, DBIL(27.7 ± 11.3) μmol/L, IBIL(178.8±14.6) μmol/L, P<0.05 was statistically significant difference between groups before treatment; newborn children of K+(3.8±0.6) mmol/L, Na+(136.8±4.2) mmol/L, Ca2+(2.2±0.39) mmol/L;newborn children after treatment K+(3.6 ±0.9) mmol/L, Na+(139.8 ±3.6) mmol/L, Ca2 +(2.3 ±0.26) mmol/L; P >0.05 had no statistically significant differences between groups. There was no significant difference in the blood coagulation function and the levels of potassium, calcium and sodium ions in the blood of newborn children with P>0.05. Conclusion Children with severe high blood bilirubin freshman implement exchange transfusion, not only can effectively help them reduce the level of bilirubin in the blood potassium ion, calcium ion and sodium ion in the blood level, can also help the newborn children to maintain electrolyte balance, blood coagulation function and signs of newborn children with no effect, high safety, is worth popularizing in clinic.
