ADC值与肿块样强化乳腺非特殊型浸润性癌预后因子相关性研究 被引量:7

Correlations between apparent diffusion coefficient and prognostic factors in non-specific type invasive breast carcinoma with mass-like enhancement

摘要 目的探讨DWI表观扩散系数(ADC)值与肿块样强化的乳腺非特殊型浸润性癌预后因子间的相关性。方法回顾分析经手术病理证实为乳腺癌患者磁共振资料,将146例磁共振增强扫描呈肿块样强化的乳腺非特殊型浸润性癌患者纳入研究,测得ADC值,并分析其与Nottingham分级系统、不同免疫组化因子表达、淋巴结是否转移的相关性。结果不同病理学分级乳腺非特殊型浸润性癌ADC值组间比较有统计学差异(P=0.001),ADC值与病理分级呈负相关(r=-0.275,P<0.05)。淋巴结转移情况、HER-2不同状态时ADC值无统计学差异(P=0.473,P=0.143),ER、PR及Ki-67不同表达状态组间ADC值有统计学差异(P=0.025、0.019、0.025),呈负相关(r值分别为-0.260、-0.253、-0.249)。结论ADC值与肿块样强化的乳腺非特殊型浸润性癌部分预后因子存在相关性。 Objective To study the correlation between the apparent diffusion coefficient (ADC) values and prognostic factors in non-specific type (NST) invasive breast carcinoma with mass-like enhancement. Methods Breast MRI of 146 patients with pathologically proven mass-like enhancing NST invasive breast carcinomas was reviewed. ADC values were correlated with prognostic factors including Nottingham grading system, expression of different immunohistochemical factors and lymph node metastasis. Results There were statistically significant differences in ADC values of tumors of different Nottingham grades (P= 0.001 ) with significant negative correlation to the pathological grading (r=-0.275, P〈0.05). There was no significant difference in ADC values of tumors with lymph node metastasis or HER-2 expression (P=0.473,P=0.143). ADC values were negatively correlated with different expressions in ER, PR and Ki-67 (P=0.025, r=-0.260; P=0.019, r=-0.253; P=0.025, r=-0.249). Conclusion ADC value correlates with prognostic factors including tumor grade, ER, PR and Ki-67 expression in NST invasive breast carcinoma with mass-like enhancement.
