MRI和X线在多发性骨髓瘤分期诊断中的应用价值分析

Application value analysis of MRI and X ray in staging diagnosis of mul-tiple myeloma

目的比较不同分期多发性骨髓瘤的MRI和X线表现,提高多发骨性髓瘤影像学诊断水平。方法选取2013年4月~2015年3月我院诊治的72例多发性骨髓瘤患者作为研究对象,均经骨髓穿刺病理活检确诊,分析所有患者的MRI和X线影像学资料,比较两种方法在多发性骨髓瘤分期诊断中的不同。结果 72例多发性骨髓瘤患者累及部位共120处,其中MRI检查出多发性骨髓瘤患者61例,检出率84.72%;检出累及部位共102处（85.00%）。X线检查多发性骨髓瘤患者46例,检出率63.89%,检出累及部位共78处（65.00%）。MRI的患者检出率和累及部位检出率显著高于X线,差异有统计学意义（P〈0.01）。X线检查分期为Ⅰ期26例,Ⅱ期28例,Ⅲ期18例;MRI检查分期为Ⅰ期14例,Ⅱ期19例,Ⅲ期39例,两种方法对于多发性骨髓瘤分期的结果比较,差异有统计学意义（P〈0.01）。结论 MRI检测较X线对于多发性骨髓瘤的检出率更高,分期诊断效果更好。

Objective To compare the MRI and X ray performance of multiple myeloma in different stages,and to improve the diagnostic level of multiple myeloma.Methods 72 cases of multiple myeloma patients in our hospital from April 2011 to March 2015 were selected and confirmed by bone marrow puncture and biopsy.MRI and X-ray imaging data of all patients were analyzed,and the difference of the two methods in the staging diagnosis of multiple myeloma was compared.Results 72 patients with multiple myeloma had 120 involved diseased regions.MRI evaluation revealed61 cases（84.72%） of multiple myeloma patients,and 102 regions（85.00%） were detected.X-ray examination revealed46 cases（63.89%） of multiple myeloma patients,and 78 regions（65.00%） were detected.The detection rate of the patients and the detection of involved sites of MRI was higher than that of the X-ray,with significant difference（P〈0.01）.The staging of X-ray examination was 26 cases of stage Ⅰ,28 cases of stage Ⅱ,18 cases of stage Ⅲ.MRI examination stage was 14 cases of stage Ⅰ,19 cases of stage Ⅱ,39 cases of stage Ⅲ,and there was significant difference in the staging of multiple myeloma between the two methods（P〈0.01）.Conclusion The detection rate of multiple myeloma in MRI is higher than that of X-ray,and the effect of staging diagnosis is better.
