医护绑定式团队中护士对出院癌症患者自我效能的影响

Effect of nurse on self efficacy in discharged cancer patients in the doctor-nurse binding team

目的探讨医护绑定式团队中护士对出院癌症患者自我管理能力和护理管理平台的影响。方法选择2014年1-12月在笔者所在医院接受治疗并出院的癌症患者88例为观察组．实施医护绑定式团队护理管理模式。选择2013年1—12月在笔者所在医院接受治疗并出院的癌症患者86例为对照组，实施常规院外护理。比较干预后两组患者的自我效能和生活质量。结果干预后观察组患者的自我效能各维度（完成自我管理、一般自我效能、达成结果、应对问题）得分明显高于对照组，差异有统计学意义（P〈0．05）；观察组与对照组在躯体功能、角色功能、气促3个维度的评分差异无统计学意义（P〉0．05），其余12个维度的评分差异均有统计学意义（P〈0．05）。两组患者护士家庭访视情况比较。患者出院1个月时差异无统计学意义（P〉0．05），出院3个月和6个月时差异均有统计学意义（P〈0．05）。结论通过实施医护绑定式团队护理管理，增加了患者的自我效能．．增强了患者自我管理水平及坚持健康行为的能力，提高了患者的生活质量。

Objective To discuss the effect of nurses on self-management ability of discharged cancer patients and nursing management platform.Methods A total of 88 cases of cancer patients were selected from January 2014 to December 2014 in our hospital for treatment and discharged as observation group, who received the implementation of the doctor-nurse binding team care management model. From January 2013 to December 2013, 86 cases of cancer patients treated in our hospital were selected as control group, who underwent the routine nursing care. The self-efficacy and quality of life of the two groups were compared after intervention. Results After intervention, each dimensions＇ score of self-efficacy （self-management completion, general self- efficacy, achieved results, problems＇ corresponding measures） in observation group was higher than that of control group after intervention （P 〈0. 05 ）; there were no statistical significance in the differences of 3 dimensions scores （body function, role function, shortness of breath） between the two groups （P〉0.05） ; the differences of other 12 dimensions scores had statistical significance （P〈0.05）. Compared the home-visit status of nurses between two groups, the difference of patients after 1 month discharge was no statistically significant （P〉0.05）, but had statistical significance 3 and 6 months after discharge （ P 〈 0.05 ）. Conclusions The implementation of doctor-nurse binding team can increase patient＇s self-efficacy and ability of self-management and health behavior persistence, as well as patient＇s quality of life.
